Abstract

Resumen: Se presenta el caso de una paciente que sufrió una herida penetrante en el ventrículo izquierdo ocasionada por un disparo con arma de fuego en el hemitórax izquierdo, sin que se deteriorase su situación hemodinámica. Dicha paciente fue evaluada mediante una tomografía axial computarizada de tórax que objetivó la presencia de un proyectil en el ápex del ventrículo izquierdo asociado a hemotórax izquierdo y varias contusiones pulmonares, sin derrame pericárdico, siendo a continuación intervenida quirúrgicamente para extraer la bala, suturar la herida en el ventrículo izquierdo y drenar el hemotórax izquierdo.Existe un porcentaje importante de pacientes con heridas penetrantes cardíacas que se presentan con buena situación clínica. Este caso resulta llamativo porque no hubo taponamiento cardíaco a pesar de la perforación cardíaca. Describimos el manejo quirúrgico y médico inicial de la rotura cardíaca. Abstract: The case is presented of a male patient with a penetrating wound in the left ventricle after being shot with a firearm in the left hemithorax. Haemodynamics were stable. A chest computed tomography revealed the presence of a projectile in the apex of the left ventricle, as well as a left haemothorax and several pulmonary contusions. There was no pericardial effusion. Surgery was performed to extract the bullet, suture the wound in the left ventricle, and drain the left haemothorax.A significant percentage of patients with penetrating cardiac wounds show good clinical status on arrival at the hospital. This case is unique as there was no cardiac tamponade despite the cardiac perforation. A description is presented of the initial surgical and medical management of the cardiac rupture.
