Reparación valvular en la insuficiencia mitral isquémica

Abstract

Resumen Existe un amplio consenso sobre la influencia negativa de la insuficiencia mitral (IM) isquemica postinfarto en la supervivencia de los pacientes. Sin embargo, aspectos como la necesidad de correccion o el momento de hacerlo se encuentran en pleno debate cientifico. Numerosos estudios sugieren que la IM residual tras revascularizacion coronaria aislada conduce a un aumento de la morbimortalidad, y empeora el pronostico de los pacientes reintervenidos. Por el contrario, el tratamiento concomitante de la cardiopatia isquemica y la IM asociada parece proporcionar mejores resultados, especialmente en terminos funcionales. No obstante, las evidencias con las que contamos actualmente no permiten establecer una clara indicacion debido a la disparidad de disenos experimentales empleados. La interrelacion entre la estructura y remodelado ventricular con la IM es muy importante en este contexto. Asi, la reparacion mitral puede contribuir al remodelado inverso del ventriculo izquierdo, y este, a la durabilidad del procedimiento, mientras que la progresion del remodelado aumenta la recurrencia de IM. La reparacion valvular mitral va dirigida a la restauracion de la coaptacion de los velos y a la eliminacion de la IM. Con objeto de pronosticar la reparabilidad de la valvula, el resultado clinico y la probabilidad de recurrencia se han definido diferentes parametros ecocardiograficos y antropometricos. A pesar de ello, la correcta integracion de los mismos y su exposicion como guia clinica de actuacion es aun un tema no resuelto. En el presente articulo, proponemos unas recomendaciones que permitan establecer un consenso sobre las indicaciones de la reparacion valvular mitral.
