Calcinose plantaire rétractile pure au cours d’une calcinose tumorale simulant une maladie de Ledderhose

Abstract

La calcinose tumorale est une affection rare caracterisee par des depots calciques dans les tissus mous periarticulaires. La localisation plantaire de cette affection est exceptionnelle et en dehors d’images radiologiques de calcifications, le diagnostic devient tres difficile. Nous rapportons l’observation d’un patient de 23 ans, suivi au sein de notre formation depuis plusieurs annees pour calcinose tumorale avec localisations multiples de masses calciques volumineuses operees avec une bonne evolution postoperatoire. Le patient presentait une retraction plantaire de l’hallux gauche evoluant depuis sept ans et devenant de plus en plus genante a la marche et au chaussage. En l’absence de signes de calcifications a la radiographie standard, le diagnostic de maladie de Ledderhose a ete retenu, bien que cette association lesionnelle (calcinose + maladie de Ledderhose) n’ait jamais ete decrite dans la litterature. Une aponevrectomie en plus d’une arthrolyse de la 1ere articulation metatarsophalangienne et arthrodese provisoire ont ete effectuees. L’examen anatomopathologique a objective des depots calciques microscopiques au sein de granulomes macrophagiques et a retenu le diagnostic de localisation plantaire de calcinose.
