Age-related features of the clinico-functional condition of patients with coronary disease

Abstract

For the purpose of evaluating the preforming factors of atherogenesis 120 patients with stable angina pectoris have been examined with an estimation of the influence of the age-specific factor in categorizing groups of adults (19-44 years) and middle age (45-64 years). It has been proved that the patients of the senior age group are characterized by more evident initial ischemia, increased levels of atherogenic β-lipoproteins, an increase of the left ventricular volumes and a decrease of the ejection fraction that is accompanied by a reliable increase of the probability of the development of eccentric and concentric hypertrophy of the left ventricle. No substantial divergences were registered in both age groups during bicycle ergometry. A greater role is played by the preforming influence of noncoronary atherogenesis at the boundary of the distribution of 45 years which is, evidently, similar in these groups, but manifestations of the development of cardiac insufficiency, the latter being corroborated by the results of echocardiography and not of bicycle ergometry.

З метою визначення преформувальних чинників атерогенезу обстежено 120 пацієнтів зі стабільною стенокардією з оцінкою впливу вікового фактору в розподілі груп дорослих (19-44 рр) та середнього віку (45-64 рр). Доведено, що пацієнти старшої вікової групи характеризувалися більш вираженою вихідною ішемією, зростанням рівнів атерогенних β-ліпопротеїнів, збільшенням об’ємів лівого шлуночка та зменшенням фракції викиду, що супроводжувалося вірогідним збільшенням ймовірності розвитку ексцентричної та концентричної гіпертрофії лівого шлуночка. За проведення велоергометрії суттєвих розбіжностей в обох вікових групах не зареєстровано. На межі розподілу 45 років більший преформувальний вплив має не коронарний атерогенез, який у цих групах є, вочевидь, подібним, а розвиток серцевої недостатності, що і доведене результатами ехокардіографії.
