Abstract

Având în vedere prevalența pe scară largă a aprovizionării insuficiente cu vitamina D, efectul dependent de doză al calcitriolului asupra imunității antituberculoase, au fost examinați 52 de copii cu vârsta cuprinsă între 1 și 14 ani la Departamentul de tuberculoză a Instituției bugetare de stat din Sankt Petersburg a Spitalului Clinic nr. 3 pentru perioada 2021ndash;2023. Criteriul de includere a fost prezența unei reacții pozitive la testul ATP și/sau testul IGRA. Criteriile de non-includere au fost bolile imunodeficienței și terapia imunosupresoare. Determinarea conținutului de calcidiol în serul sanguin al pacienților a relevat: deficit de vitamina D la 19,2%, deficit moderat la 40,4%, deficit sever la 32,6% dintre pacienți. Conținutul mediu de calcidiol în serul sanguin al tuturor pacienților a fost de 15,71plusmn;1,13 ng/ml. La copiii cu infecție tuberculoasă cu semne de activitate conform testelor imunologice în prezența deficienței de vitamina D conținut de calcidiol mai mic de 20 ng/ml, probabilitatea diagnosticării formelor active de tuberculoză este mai mare decât în cazul deficitului și nivelurilor normale ale acestei vitamine. Taking into account the widespread prevalence of insufficient supply of vitamin D, the dose-dependent effect of calcitriol on anti-tuberculosis immunity, 52 children aged 1-14 years were examined at the tuberculosis department of St. Petersburg State Budgetary Institution of St. Petersburg Childrenrsquo;s Infectious Hospital No. 3 for the period 2021ndash; 2023. The inclusion criterion was a positive reaction to the RTA test and/or IGRA test. Exclusion criteria included immunodeficiency diseases and immunosuppressive therapy. Determination of the calcidiol content in the blood serum of patients revealed the following: vitamin D deficiency in 19.2%, moderate deficiency in 40.4%, and severe deficiency in 32.6% of patients. The average calcidiol content in the blood serum of all patients was 15.71plusmn;1.13 ng/ml. In children with tuberculosis infection with signs of activity according to immunological tests in the presence of vitamin D deficiency calcidiol content less than 20 ng/ml, the probability of diagnosing active forms of tuberculosis is higher than in the case of deficiency and normal content of this vitamin OR= 8,05 DI [2,03-32,01] Se = 0,71; RR = 1,86 DI [1,14- 3,03] Se = 0,24; F = 0,00285, phi;c=0,440.

Учитывая широкую распространённость недостаточной обеспеченности витамином D, дозозависимое воздействие кальцитриола на противотуберкулезный иммунитет, на базе туберкулезного отделения СПб ГБУЗ ДИБ №3 обследовано 52 ребенка в возрасте 1-14 лет за период 2021ndash;2023гг. Критерием включения было наличие положительной реакции на пробу с АТР и/или IGRA-теста. Критериями невключения являлись иммунодефицитные заболевания, проведение иммуносупрессивной терапии. Определение содержания кальцидиола в сыворотке крови пациентов выявило: недостаточность витамина D у 19,2%, умеренно-выраженный дефицит ndash; у 40,4%, тяжелый дефицит ndash; у 32,6% пациентов. Среднее содержание кальцидиола в сыворотке крови всех пациентов составило 15,71plusmn;1,13 нг/мл. У детей с туберкулезной инфекцией с признаками активности по иммунологическим пробам при наличии дефицита витамина D содержание кальцидиола менее 20 нг/мл вероятность диагностики активных форм туберкулеза выше, чем при недостаточности и нормальном содержании данного витамина OR= 8,05 ДИ [2,03- 32,01] Se = 0,71; RR = 1,86 ДИ [1,14- 3,03] Se = 0,24; F= 0.00285, phi;c=0,440.
