Диагностика вирусной инфекции Эпштейна-Барр при системной красной волчанке и ревматоидном артрите

Abstract

There are presented data of the literature on the molecular biological properties of Epstein-Barr virus (EBV), mechanisms of infection persistence and reactivation of viral infection. EBV was shown to be able to participate in the development of autoimmune processes via polyclonal activation of B-lymphocyte, of deficiency CD8-T-lymphocytes, molecular mimicry, modifications of cellular antigens and activation of the superantigen and adjuvant effect. In patients with systemic lupus erythematosus there is revealed an increase in EBV DNA in peripheral blood mononuclear cells, the weakening of EBV specific T-cell response, the impairment of the expression of viral lytic genes and the rise in the EBV-specific antibodies level. In rheumatoid arthritis there is detected the EBV gene expression in the synovial membrane, an increase in the viral load and the elevation of the level of EBV-specific antibodies.

Представлены данные литературы о молекулярно-биологических свойствах вируса Эпштейна-Барр (ВЭБ), механизмы инфицирования, персистенции и реактивации вирусной инфекции. Показано, что ВЭБ может принимать участие в развитии аутоиммунных процессов за счет поликлональной активации В-лимфоцитов, дефицита CD8 Т-лимфоцитов, молекулярной мимикрии, модификации клеточных антигенов, активации суперантигена и адъювантного эффекта. У больных системной красной волчанкой выявляется увеличение ДНК ВЭБ в мононуклеарах периферической крови, ослабление ВЭБ-специфического Т-клеточного ответа, нарушение экспрессии вирусных литических генов и повышение уровня ВЭБ-специфических антител. При ревматоидном артрите определяется экспрессия генов ВЭБ в синовиальной оболочке, увеличение вирусной нагрузки и повышение уровня ВЭБ-специфических антител.
