Воспалительные псевдоопухоли желудочно-кишечного тракта у детей

Abstract

An inflammatory pseudotumor is difficult enough to diagnose and treat because of the similarity of its clinical picture and that of malignant neoplasms. The author discusses specific features of this pathology in children on the basis of data presented in literature and his own observations. In children observed by the author there were syndromes of a palpable tumor in the abdominal cavity, pain and intoxication syndrome. Blood tests revealed hyperleukocytosis with the left stab neutrophil shift and enhanced ESR, high indices of CRP and sialic acids, a higher level of neuron-specific enolase (23.1 ± 5.8 ng/ml) and LDH (543 ± 61.6 IU/I). In two children of three, the underlying factor of the inflammatory pseudotumor was a vascular malformation of the intestines. In one child the pseudotumor was located in the ileocecal angle, in one child in the small intestine, and also in one patient in the sigmoid colon. All children underwent radical operative interventions. In the postoperative period, apart from antibacterial, infusion and anti-inflammatory therapy, a polyenzyme preparation was administered, which did not only promote the resolution of the indurative component of the pseudotumor, but ensured a marked antiinflammatory and antiallergic effect.

Воспалительная псевдоопухоль довольно трудна для диагностики и лечения из-за сходства ее клинической картины со злокачественными новообразованиями. В статье обсуждаются, на основе данных литературы и собственных наблюдений, особенности этой патологии в детском возрасте. У наблюдаемых автором детей отмечены синдромы пальпируемой опухоли в брюшной полости, болевой и интоксикационный. При исследованиях крови выявлены гиперлейкоцитоз с палочкоядерным сдвигом влево и увеличением СОЭ, высокие значения СРБ и сиаловых кислот, повышенный уровень нейрон-специфической енолазы (23,1 ± 5,8 нг/мл) и ЛДГ (543 ± 61,6 МЕ/л). У двоих детей из трех в основе воспалительной псевдоопухоли лежала сосудистая мальформация кишечника. Псевдоопухоль локализовалась в илеоцекальном углу у одного ребенка, в тонкой кишке у одного и в сигмовиднай кишке также у одного пациента. Всем детям были выполнены радикальные оперативные вмешательства. В послеоперационном периоде, кроме антибактериальной, инфузионной и противовоспалительной терапии был применен полиферментный препарат, который не только способствовал рассасыванию индуративной составляющей псевдоопухоли, но и обеспечивал выраженный противовоспалительный и противоаллергический эффект.
