Органические психические расстройства в МКБ-10: несовершенство критериев диагностики или псевдодиагностические категории

Abstract

В статье обзорного характера затрагиваются вопросы несовершенства диагностических критериев органических психических расстройств на примере органического аффективного расстройства в МКБ-10. Подчеркивается, что приводимые в МКБ-10 критерии диагностики носят самый общий характер и не позволяют на феноменологическом уровне без учета анамнеза отграничить органические психические расстройства от сходных в психопатологическом отношении категорий неорганической (эндогенной) природы. Приведены литературные данные, показывающие, что некоторые так называемые экзогенные и соматогенные факторы имеют значение для патогенеза так называемого большого депрессивного эпизода, что говорит об общности патогенеза органических и неорганических аффективных расстройств. Исходя из литературных данных, предполагается, что так называемая стадия «эндогенных» проявлений органической депрессии является собственно самой эндогенной депрессией, которая в последующем стирается текущим органическим процессом.

This review article approaches the issue of insufficient diagnostic criteria for organic mental disorders using as an example ‘organic affective disorder’ in the ICD-10. The author emphasizes that the ICD-10 diagnostic criteria appear to be very general and do not allow to distinguish on phenomenological level, without personal history data, organic mental disorders from similar ones in terms of psychopathology belonging to categories of non-organic (endogenous) origin. The article provides literary data that show that some of the so called exogenous and sotamogenic factors can play an important role in pathogenesis of the so called major depressive episode, thus pointing to overlaps in pathogenesis of organic and non-organic affective disorders. On the basis of literary data, the author suggests that the so called ‘endogenous’ stage of organic depression happens to be the endogenous depression itself and this picture erodes with time due to current organic process.
