Abstract

С целью определения места заболеваний органов пищеварения в структуре соматической патологии у подростков с диффузным нетоксическим зобом обследовано 145 девочек и 177 мальчиков 10-17 лет с зобом. Проводили ультразвуковое исследование щитовидной железы, изучали концентрацию тиреотропина и свободных фракций тиреоидных гормонов. Для диагностики сопутствующей соматической патологии проводили расширенное комплексное обследование с определением показателей липидного спектра крови. Установлено, что большинство подростков (87,0 %) имели два и более сопутствующих соматических заболевания. Хроническую патологию пищеварительной системы диагностировали у 81,5 % обследованных, среди которых преобладали функциональные нарушения со стороны желчного пузыря по гипотоническому типу (76,3 %). У подростков с зобом диагностированы патологические изменения в липидном спектре за счет увеличения концентрации бета-липопротеидов (51,9 %), общего холестерина (39,9 %) и холестерина липопротеидов низкой плотности (23,7 %), которые были наиболее характерны для больных с неблагоприятным прогнозом течения тиреопатии. Подтверждена тесная взаимосвязь между показателями липидного спектра и тиреоидного профиля.

145 girls and 177 boys aged 10-17 years were examined to determine a place of digestive diseases in the structure of somatic pathology in adolescents with nontoxic diffuse goiter. Thyroid ultrasound has been carried out, concentrations of thyroid-stimulating hormone and free fractions of thyroid hormones have been measured. To diagnose concomitant somatic pathology, the authors have performed an extensive complex examination with determination of blood lipids. It is found that most adolescents (87.0 %) had two or more concomitant somatic diseases. Chronic digestive pathology has been detected in 81.5 % of patients, with domination of functional disorders of the gallbladder by a hypotonic type (76.3 %). In adolescents with goiter, we have diagnosed pathological changes in blood lipids due to increased concentrations of beta-lipoproteins (51.9 %), total cholesterol (39.9 %) and low-density lipoprotein cholesterol (23.7 %), which are most characteristic of patients with unfavorable course of thyropathy. Our study has confirmed a close relationship between the blood lipids and thyroid profile.
