Применение современных электрокардиографических технологий у детей и подростков с артериальной гипертензией

Abstract

Authors had conducted investigation 129 patients 11-17 years old with arterial hypertension and 37 healthy children. Analysis of signal-averaged ECG showed what children and adolescents with arterial hypertension have alteration of depolarization in the shape of increase parameter TotQRS and availability late ventricular potentials. Increase of dispersion interval QT, its corrected index and dispersion interval QTapex testify about heterogeneity ventricular repolarization. Pathological pattern (late ventricular potentials in combination with increase of dispersion interval QT) is characterized for children with prolonged flowing disease and availability modification geometric model left ventricular. Employment contemporary easy, low-spending and noninvasive diagnostic technologies appear rational in opportune revealing unfavorable electrophysiological and structure geometric myocardial alterations of left ventricular in children and adolescents with arterial hypertension what dictate need its establishment to clinical practice.

Обследованы 129 пациентов в возрасте 11-17 лет с артериальной гипертензией (АГ) и 37 здоровых детей и подростков в качестве контрольной группы. Анализ электрокардиографии высокого разрешения показал, что у пациентов с АГ имеются изменения деполяризации в виде увеличения временного параметра TotQRS, наличия поздних потенциалов желудочков. О гетерогенности реполяризации желудочков у них свидетельствует увеличение дисперсий интервала QT, его корригированного индекса и интервала QTapex. Патологический паттерн (наличие поздних потенциалов желудочков в сочетании с увеличением дисперсии интервала QT) характерен для детей с длительно текущим заболеванием и наличием изменений геометрической модели левого желудочка (ЛЖ). Применение современных простых, малозатратных и неивазивных диагностических технологий является рациональным в своевременном выявлении неблагоприятных электрофизиологических и структурно-геометрических изменений миокарда ЛЖ у детей и подростков с АГ, что диктует необходимость их внедрения в клиническую практику.
