Изменения показателя транскапиллярного обмена у больных с признаками абдоминального сепсиса в процессе жидкостной ресусцитации

Abstract

152 patients with abdominal sepsis were investigated on fluid body volumes during fluid resuscitation with different plasma substitutes by means of two-frequency impedance method. Quantity of transcapillary exchange index (TEI) was calculated in these patients too. TEI increased from the beginning to the end of investigation in patients with only crystalloid fluid support and decreased in all groups of patients when colloid plasma substitutes has been used. There was no direct dependence between TEI change and severity of the manifestation of capillary leak syndrome. The intravascular fluid loss to interstitial sector and TEI decrease were attained more effectively, when infusions of hydroxiethyl starch derivatives has been used.

У 152 пациентов с абдоминальным сепсисом двухчастотным импедансным методом проведено исследование объемов водных пространств и секторов организма в процессе жидкостной ресусцитации с использованием разных групп плазмозаменителей и расчетом показателя транскапиллярного обмена (ПТО). ПТО возрастал от начала до конца исследования у больных, которые получали исключительно кристаллоидные растворы, и уменьшался во всех группах больных при использовании коллоидных растворов. Прямой зависимости между изменениями ПТО и тяжестью проявлений синдрома капиллярной утечки обнаружено не было. Уменьшение потери жидкости из сосудов в интерстиций и снижение ПТО наиболее эффективно достигались при использовании производных гидроксиэтилкрахмала.
