Abstract

Проведены экспериментально-клинические исследования изменения функциональных показателей, состояния микроциркуляции и гемостатических нарушений при позвоночно-спинальной травме. Выявлено, что компрессионный перелом тела 1 поясничного позвонка вызывает значительные сдвиги в изучаемых системах уже через 30 минут после травмы: снижение артериального давления и кровотока в жизненно важных органах, спазм и малокровие капилляров, открытие артерио-венозных анастомозов, венозное полнокровие, стазы, сладжи эритроцитов и т.д. Неадекватность кровотока влечет за собой гипоксию и активацию свертывающих компонентов крови при депрессии фибринолитического и противосвертывающего звеньев плазменного гемостаза. Совокупность гемостатических нарушений свидетельствует об опасности развитич ДВС-синдрома. Для нормализации и улучшения метаболизма и предупреждения необратимых расстройств в жизненно важных системах был применен препарат глиатилин. Введение этого холиномиметика интактным животным не вызывает патологических сдвигов в системах микроциркуляции и гемостаза. При использовании его в качестве корректора вышеперечисленных нарушений при позвоночно-спинальной травме выявлена незначительная активация свертывающих компонентов крови с компенсаторной нормализацией противосвертывающего и фибринолитического звеньев системы гемостаза. Глиатилин снижает риск тромбогемор-рагических осложнений и увеличивает выживаемость животных в посттрпавматическом периоде. Применение глиатилина в клинике для лечения больных с тяжелой позвоночно-спинальной травмой в комплексе лечебных мероприятий способствует регрессу неврологической симптоматики и сокращает сроки реабилитации больных.

Experimental and clinical research of changes of the functional parameters of microcirculation and hemostatic disorders in spine-spinal cord injury is carried out. It is revealed that compression fracture of the body 1 of lumbar vertebra causes significant shifts in the studied systems as early as 30 minutes after injury: lowering of blood pressure and blood flow to vital organs, spasm and anemia of capillaries, opening of arterio-venous anastomoses, venous plethora, stasis, sludge of erythrocytes, etc. Inadequate blood flow entails hypoxia and activation of blood components coagulation with depression of fibrinolytic and anticoagulation links of plasma hemostasis. The combination of hemostatic disorders indicates the danger of DIC-syndrome. For normalization and improvement of metabolism and prevention of irreversible disorders in vital systems, the drug gliatilin was applied. The introduction of this holinomimetics to intact animals does not cause pathological changes in the systems of microcirculation and hemostasis. When used for the correction of the above spine-spinal injuries, a slight activation of the coagulation of blood components was revealed with compensatory normalization of anticoagulant and fibrinolytic links of hemostasis. Gliatilin reduces the risk of thrombohemorrhagic complications and increases the survival rate of animals in posttraumatic period. The use of gliatilin in the clinic for treatment of patients with severe spine with spinal injuries in the complex treatment leads to regress of neurological symptoms and reduces the rehabilitation period of patients.
