Abstract

Важность психосоциальных проблем при сахарном диабете (СД) признана давно. Пионеры в этой области расценивали психосоциальный фактор как один из этиологических. С течением времени эта точка зрения была пересмотрена, и сейчас психосоциальные факторы принято расценивать в качестве как причин, так и следствий проблем, возникающих у пациента в его взаимоотношениях со своей болезнью. Психические изменения при эндокринных заболеваниях полиморфны, однако их развитие подчиняется определенным закономерностям. Эти закономерности сводятся к развитию на ранних стадиях и при относительно доброкачественном течении болезни так называемого психоэндокринного, или психопатоподобного (?эндокринного психосиндрома? ? по M.Bleuler); по мере прогрессирования заболевания этот синдром постепенно переходит в психоорганический (амнестикоорганический). На фоне указанных синдромов, обычно в связи с нарастанием тяжести эндокринного страдания, могут развиваться острые или затяжные психозы. Своеобразие психических нарушений при отдельных заболеваниях определяется преобладанием тех или иных расстройств в структуре синдрома.
