Abstract

[ES] El cáncer vesical es una de las neoplasias más comunes en varones de países desarrollados y presenta una elevada tasa de recurrencia, lo que incrementa considerablemente su impacto clínico y económico. La participación del sistema inmunitario en la evolución del tumor, así como su interacción con el microambiente tumoral, ha cobrado creciente interés en los últimos años. Los neutrófilos, primera línea de defensa en el sistema inmunitario, parecen regular el proceso tumoral a través de la formación de neutrophil extracelular traps (NETs). En este contexto, el objetivo del presente trabajo es estudiar la posible implicación de ciertas citoquinas plasmáticas, moléculas clave en la regulación del sistema inmunitario, en la desregulación del proceso de NETosis observada previamente en pacientes con cáncer vesical. Para ello, en una cohorte de 14 pacientes con cáncer vesical (7 con tumor músculo infiltrante (MIBC) y 7 con tumor no músculo infiltrante (NMIBC)) y 7 controles sanos, se llevó a cabo la cuantificación de un perfil de 80 citoquinas plasmáticas mediante un array prediseñado. Posteriormente, se seleccionaron cuatro citoquinas candidatas, en base a la presencia de diferencias significativas entre grupos (P 0,85, P < 0,05), para su validación mediante ensayos ELISA en una cohorte ampliada (48 pacientes y 24 controles): GRO-945;, leptina, SCF e IGFBP-1. En la cohorte de validación, observamos unos niveles más elevados de GRO-945; en pacientes con cáncer vesical frente a controles, especialmente en aquellos con el subtipo MIBC. En la leptina, se observaron niveles inferiores en pacientes MIBC en comparación con los grupos NMIBC y control. Por otro lado, SCF presentó diferencias entre grupos en el ensayo de array que no fueron validadas por ELISA, mientras que IGFBP-1 no mostró diferencias significativas entre grupos, a pesar de una capacidad discriminativa moderada en el análisis de curvas ROC (A.U.C=0,8776, P=0,018, IC=0,7-1) En conjunto, estos hallazgos apoyan la implicación de determinadas citoquinas en la disregulación del proceso de NETosis en cáncer vesical, con especial relevancia de GRO-945;.
