Abstract

OBJETIVO: se hace una revisión de las diferentes alternativas y los anticonceptivos considerando las características especiales de su posible interacción medicamentosa con los fármacos anticonceptivos, así como sobre su potencia anticonceptiva y la seguridad en la mujer con epilepsia. DESARROLLO: se revisan la biodisponibilidad, el metabolismo, las dosis recomendadas, las interacciones medicamentosas, y la seguridad. Se revisan los aspectos relacionados con el efecto de los componentes hormonales de los anticonceptivos más utilizados en el curso de la epilepsia. Se hacen recomendaciones prácticas de planificación familiar en este grupo de mujeres. CONCLUSIONES: la consejería sobre anticoncepción en mujeres con epilepsia representa un reto especial para neurólogos y ginecólogos. El conocimiento claro de cada una de las diferentes alternativas en este grupo de pacientes, de sus riesgos y variaciones en su uso, permitirán una aproximación adecuada a estas pacientes ofreciéndoles métodos seguros y confiables.

OBJECTIVE: in order to establish the most appropiated contraceptive methods for epileptic women, a review of the different alternatives is made. Considerations about interactions between contraceptives and antiepilepctic drugs, safety issues and contraceptive efficacy in this kind of patient are also made. DEVELOPMENT: different alternatives on contraception in the epileptic women are reviewed, including biodisponibility, metabolism, recommended doses, drug interaction and safety topics. The influence of hormonal contraceptives on epilepsy is also taken into account. Practical recommendations about contraception in this group of patients are made. CONCLUSIONS: contraceptive counselling for epileptic women is an important challenge for neurologists and gynecologists. A comprehensive knowledge of the several alternatives, their risks and special considerations is necessary in order to offer them a safe and reliable method.
