Abstract

Introduction : L’instauration d’un formulaire thérapeutique de médicaments essentiels basé sur les directives thérapeutiques standards est le levier d’une prescription médicale de qualité dans un centre hospitalier. L’objectif de ce travail était d’évaluer la conformité des prescriptions médicales selon le formulaire thérapeutique précédemment mis en place. Méthodologie : Il s’agissait d’une étude transversale à visée descriptive qui s’est déroulée du 1er septembre au 31 octobre 2024. La collecte des données a été réalisée sur 1348 ordonnances prescrites aux patients dans les différents services du centre hospitalier universitaire (CHU) Pr Bocar Sidy Sall de Kati. Les données collectées ont été saisies et analysées sur le logiciel Microsoft Excel. Résultats : Les ordonnances étaient prescrites par les médecins spécialistes (n=502) et généralistes (n=393). La qualité de prescription avait obtenu un score total de 6,56 sur 8 points. Les protocoles de traitements mis en en oeuvre étaient conformes à ceux établis pour les services avec un score de 1,57 sur 2 points. Les médicaments prescrits étaient conformes au formulaire thérapeutique établi (94%), à la liste nationale des médicaments essentiels du Mali version 2019 (74%) et version 2024 (89%). Conclusion : Notre étude a montré une bonne conformité des prescriptions au formulaire thérapeutique mis en place. Elle pourrait donc présager d’une mise à échelle de cette initiative dans les autres CHU du Mali et dans les établissements hospitaliers régionaux. Introduction: The introduction of an essential drug formulary based on standard therapeutic guidelines is the key to high-quality medical prescribing in a hospital centre. The aim of this study was to assess the compliance of medical prescriptions with the therapeutic formulary previously introduced. Methodology: This was a descriptive cross-sectional study which took place from 1 September to 31 October 2024. Data were collected on 1348 prescriptions given to patients in the various departments of the Pr Bocar Sidy Sall University Hospital Centre (CHU) in Kati. The data collected was entered and analysed using Microsoft Excel software. Results: The prescriptions were given by specialists (n=502) and general practitioners (n=393). Prescribing quality obtained a total score of 6.56 out of 8 points. The treatment protocols implemented complied with those established for the departments, with a score of 1.57 out of 2 points. The medicines prescribed complied with the established therapeutic formulary (94%), the national list of essential medicines in Mali, version 2019 (74%) and version 2024 (89%). Conclusion: Our study showed that prescriptions complied well with the therapeutic formulary that had been introduced. It could therefore be used as a basis for scaling up this initiative in Mali's other university hospitals and regional hospitals.
