Caries prevalance among children with congenital and acquired heart diseases

Abstract

One of the important complications that might be seen among the children with congenital and acquired heart diseases is infective endocarditis which has 25-30% mortality risk. All cardiac malfunctions resulting from the hemodynamic trauma of the turbulence of blood stream prepares a sound basis for infective endocarditis. Infective endocarditis develops due to the infusion of bacteria from local areas to the damaged region via blood. Eighty-two children who were in routine control in Ege University School of Medicine Department of Pediatric Cardiology were included in this study. Twenty-eight patients having acquired- and 32 children having congenital heart disease were included in the study group. There were 22 healthy children in the control group. Caries prevalence and DMF index of the acquired and congenital heart diseased children were determined via intraoral examination. The results showed that caries prevalence and DMF index of the study group were considerably higher compared to the healthy children. It is important to remove the microbial dentinal plaque which is effective on the caries and periodontal disease that may lead to the bacteremia especially in children having cardiac disorders.

Gerek konjenital gerekse akkiz kalp hastalığı taşıyan çocuklarda ortaya çıkabilen ve %25-30 oranında mortalite ile sonuçlanan önemli bir komplikasyon da infektif endokardittir. İnfektif endokardit konjenital veya akkiz kalp hastalığına bağlı türbülansın yol açtığı endokardiyal hasara, vücuttaki herhangi bir infeksiyon odağından kana geçen bakterilerin tutunmasıyla ortaya çıkar. Bu çalışmaya Ege Üniversitesi Tıp Fakültesi Pediatrik Kardiyoloji Bilim Dalı’nda rutin kontrolleri yapılan 82 çocuk dahil edildi. Çalışmaya dahil edilen çocuklardan 28’i akkiz, 32’si konjenital kalp hastası idi. Tamamen sağlıklı 22 çocuk ise kontrol grubunu oluşturdu. Çocukların yapılan ağız muayeneleri sonrasında DMF indeksleri ve çürük prevalans hızları belirlendi. Sonuçta konjenital ve/veya akkiz kalp hastalığı taşıyan çocukların DMF indeksleri ve çürük prevalans hızlarının kontrol grubununkinden anlamlı derecede yüksek olduğu bulunmuştur. Bu çalışmanın sonucuna göre çürük ve periodontal hastalıkların oluşumunda etkili ve direkt bir bakteriyemi sebebi olan mikrobiyal dental plağın kaldırılmasının, konjenital ve akkiz kalp hastalığı taşıyan çocuklarda özellikle önemli olduğu düşünülmüştür.
