Plain radiography findings in patients with low back pain

Abstract

The aim of this study was to investigate the radiopathological findings based on plain radiographic examinations of patients with low back pain. Of the 747 patients, with a mean age of 47.97 years, 488 (65.32%) were women and 259 (34.67%) were men. Plain antero-posterior and lateral lumbosacral vertebrae radiographes were obtained from all patients and were examined in regard to the presence of congenital abnormalities (sacralisation, lumbarisation, spina bifida), lumbar degenerative disc disease, osteoporosis, spondylolisthesis and scoliosis. It was found that degenerative disc disease (29.58%) was the most frequently encountered radiopathological finding, followed by osteoporosis (23.03%) and congenital abnormalities (21.15%). Spondylolisthesis (6.02%) and scoliosis (4.28%) were relatively rare. The most common congenital abnormality was sacralisation (17.67%). Degenerative disc disease was most frequently seen al L4-L5 and L5-S1 levels. When the radiopathological findings were analyzed according to sex distribution, there was no significant difference between women and men, except for osteoporosis. Conventional radiographic examination has been useful to give proper exercise protocol and brace for patients with spondilolisthesis and to investigate secondary factors, in young patients with osteoporosis.

Bu çalışmanın amacı, bel ağrısı olan hastalarda direkt radyolojik incelemedeki radyopatolojik bulguları araştırmaktı. Ortalama yaşlan 47.97 yıl olan 747 hastanın 488'i kadın (%65.32), 259'u erkek (%34.67) idi. Tüm hastaların antero-posterior ve lateral lumbosakral vertebra radyografileri elde edildi. Direkt radyografiler, konjenital anomaliler (sakralizasyon, lumbalizasyon, spina bifida), lomber dejeneratif disk hastalığı, osteoporoz, spondilolistezis ve skolyoz varlığı yönünden değerlendirildi. Dejeneratif disk hastalığının (%29.58) en sık karşılaşılan radyopatolojik bulgu olduğu, bunu osteoporoz (%23.03) ve konjenital anomalilerin (%21.15) izlediği saptandı. Spondilolistezis (%6.02) ve skolyoz (%4.28) nisbeten daha nadirdi. En sık görülen konjenital anomali sakralizasyondu (%17.67). Dejeneratif disk hastalığı en sık L5-S1 ve L4-L5 düzeylerinde görüldü. Bu radyopatolojik bulguların cinse göre dağılımı incelendiğinde, osteoporoz dışındaki bulguların kadın ve erkek hastalarda görülme sıklıklarında anlamlı bir fark olmadığı görüldü. Konvansiyonel radyografik inceleme yapılması, spondilolistezisli hastalarda, uygun egzersiz protokolü ve korsenin verilmesi ve osteoporozlu genç hastalarda sekonder faktörlerin araştırılması için yararlıdır.
