We propose a global initiative providing critical decision support to growing urban centers on human and environmental health impacts of peri-urban agriculture. Agricultural practices impact human health via nutrition pathways (dietary intakes, malnutrition, diet related non-communicable diseases), disease pathways (water/air quality, chemical exposure, vector borne diseases), and their interactions. Agricultural practices impact environmental health, with feedback loops between environmental services and agriculture. We provide a systems approach jointly owned and developed with the leadership of eight municipalities representing a global gradient from less to more developed contexts. With these cities we define principal synergies and trade-offs between human and environmental health as mediated by land use change (trends, nature, location and management). We focus on food consumption and land use scenarios to identify solutions for: (1) what food is consumed and where it is sourced from, (2) what food is produced locally and where it is consumed, and (3) how and where local food is produced, as the three production-consumption drivers of human and environmental health. With the partner municipalities we develop and test on the ground demand-driven intervention options for influencing food choices and land use planning in support of producing healthy food in a sustainable fashion.
