Clinical findings are very similar to inguinal hernia. Patients usually present with groin pain and groin swelling that can be enlarged by the Valsalva maneuver. Since management is different, it is important to make a differential diagnosis of these two diseases. Özet Round ligament varisleri, gebelikte nadir görülen bir sağlık sorunu olup klinik bulgular kasık fıtığı ile oldukça benzerlik göstermektedir. Hastalar genellikle kasık ağrısı ve Valsalva manevrası ile büyüyen kasık şişliği şikayetiyle başvurmaktadır. Klinik yönetimi inguinal herniden farklı olduğu için bu iki hastalığın ayırıcı tanısının yapılması önemlidir.

This is most likely related to the use of spinal or epidural anesthesia and is very rare after general anesthesia. In this article, we aimed to present a case of acute transverse myelitis that developed after laparoscopic total gastrectomy under general anesthesia. Özet Cerrahi müdahaleden sonra akut transvers miyelit nadir görülen bir komplikasyondur. Bu durumun öncelikle spinal veya epidural anestezi ile ilişkili olabileceği düşünülmekte olup genel anesteziden sonra çok nadir görülmektedir. Bu yazıda genel anestezi eşliğinde laparoskopik total gastrektomi sonrası gelişen akut transvers miyelit olgusunu sunmayı amaçladık.

Entecavir, a guanosine nucleoside analog (NA), is a commonly used and generally safe therapeutic agent in HBV infection. In this paper, we aimed to present a case of chronic HBV that developed severe myopathy as an extremely rare side effect after the use of entecavir. Patients receiving entecavir therapy for chronic HBV should be closely monitored for the development of myopathy as well as other known and common side effects. Özet Kronik Hepatit B virusu (HBV) enfeksiyonu, dünya çapında milyonlarca insanı etkileyen ve karaciğer yetmezliği ve hepatoselüler karsinoma gibi ciddi sonuçlara neden olabilen kronik viral bir hastalıktır. Bir guanozin nükleozit analoğu (NA) olan entekavir, HBV enfeksiyonunda yaygın olarak kullanılan ve genellikle güvenli bir terapötik ajandır. Bu yazıda, entekavir kullanımından sonra son derece nadir bir yan etki olarak şiddetli miyopati gelişen bir kronik HBV olgusunu sunmayı amaçladık. Kronik HBV için entekavir tedavisi alan hastalar, miyopati gelişimi ve diğer bilinen ve yaygın yan etkiler açısından yakından izlenmelidir.

Bu tanımlayıcı anket çalışması, Türkiye'de Mart 2021 ve Haziran 2021 tarihleri arasında aile hekimliği polikliniklerinde yetişkinlerle yürütülmüştür. Anket 43 soru içeriyordu; ilk bölümde katılımcılardan bazı sosyodemografik verileri sağlamaları istendi; ikinci bölümde ise mevcut sigara içme durumu, kronik hastalık varlığı, herhangi bir kanser türünün olup olmadığı, ailede kanser öyküsü, ülkesindeki kanser tarama programı hakkında bilgi alma durumu, COVID-19 salgını öncesinde ve sırasında herhangi bir kanser taraması yaptırma öyküsü anketin üçüncü bölümü ise ulusal kanser tarama programı (kolorektal kanser, serviks kanseri, meme kanseri) ile ilgili sorulara odaklandı. Ankete katılan 191 kişiden %54.2'si pandeminin dışkıda gizli kan testi yaptırma sıklığını etkilediğini, %47.4'ü pandeminin kolonoskopi sıklığını etkilediğini, %37.7'si pandeminin mamografi çekme sıklığını etkilediğini ve %37.1'i pandeminin pap smear testi yaptırma sıklığını etkilediğini belirtti. Bu gecikmelerin en çok belirtilen nedeni enfeksiyon kapma endişesiyle sağlık ocağına gitmekten korkmaktı. Kanser taramasındaki kesintiler, mevcut kanserlerin geç teşhisine, kanserle ilişkili ek ölümlere yol açacaktır. Tarama oranları COVID öncesi seviyelere dönene kadar bu kesintilerin büyük bir etkisi olabilir. Sonuçlarımız pandeminin tarama programları üzerine olumsuz etkilerini gösterdi ve en çok vurgulanan neden enfeksiyon kapma endişesiyle sağlık ocağına gitmekten korkmaktı.

Doğada genellikle kuyu suyu, böceklerin bağırsakları ve sürüngenlerde bulunan Y. regensburgei insanlarda septik artrit, yumuşak doku enfeksiyonu ve otitis eksterna gibi farklı tiplerde enfeksiyonlara neden olabilmektedir. Bu yazıda Y. regensburgei’nin neden olduğu ve travmaya bağlı gelişen bir yumuşak doku enfeksiyonu vakası sunulmuştur. Abstract Yokenella regensburgei is a fermentative, motile, oxidase-negative, gram-negative bacillus belonging to the Enterobacteriaceae family and is an opportunistic human pathogen that is caused infections mainly in immunocompromised patients and rarely in immunocompetent individuals. Y. regensburgei, which is generally found in well water, insect guts, and reptiles in nature, can cause different types of infections such as septic arthritis, soft tissue infection, and otitis externa in humans. In this article, a case of soft tissue infection caused by Y. regensburgei and due to trauma is presented.

It has clinical importance associated with substantial morbidity and mortality in malaria-endemic regions. We report a young patient who developed acute renal injury as a result of severe malaria. In this case, malaria diagnosis was confirmed using the malaria rapid screening test and microscopic inspection of blood smears. Fever, nausea and vomiting, loss of appetite and jaundice were the clinical presentations. Treatment with artemether injection, volume expansion and antipyretics was given. He recovered well without needing hemodialysis. This is not always the case, and the prognosis is dependent on the time of presentation, the diagnosis, and the treatment received. Özet Akut böbrek hasarı, ciddi sıtmanın nispeten nadir görülen bir komplikasyonudur. Sıtmanın endemik olduğu bölgelerde morbidite ve mortalite ile ilişkili klinik önemi vardır. Şiddetli sıtma nedeniyle akut böbrek hasarı gelişen genç bir hasta olgusunu sunuyoruz. Bu olguda, sıtma tanısı sıtma hızlı tarama testi ve kan yaymalarının mikroskobik incelemesi ile doğrulandı. Klinik prezentasyonlar ateş, bulantı ve kusma, iştahsızlık ve sarılıktı. Artemeter enjeksiyon ile tedavi edilen hastaya volüm desteği ve antipiretik verildi. Hasta hemodiyaliz gereksinimi olmaksızın iyileşti. Olgularda durum her zaman böyle değildir ve prognoz başvuru zamanına, tanıya ve alınan tedaviye bağlıdır.

Abstract Tularemia is a zoonotic bacterial infection caused by Francisella tularensis with diverse clinical presentations. We present an oropharyngeal tularemia case complicated with retropharyngeal abscess which is a rare reported complication of tularemia. A twenty-one-year old male farmer was admitted to our hospital with a history of fever, headache, generalized aches and tender lump on the left side of neck. Physical examination revealed tender lymphadenopathy on the left posterior cervical region and retropharyngeal abscess. Surgical intervention was performed under general anesthesia. The diagnosis was made by serological tests. After antimicrobial therapy the patient recovered completely without sequelae. Özet Tularemi Francisella tularensis’in etken olduğu, çeşitli klinik tablolar ile prezante olan bir zoonotik bakteriyel enfeksiyondur. Bu yazıda tulareminin ender olarak raporlanmış bir komplikasyonu olan retrofarengeal apseli bir orofarengeal tularemi olgusu sunulmuştur. Yirmi bir yaşında erkek hasta ateş, baş ağrısı, yaygın vücut ağrısı ve boyun sol yanında boyunda şişlik şikayetleri ile hastanemize başvurdu. Fizik muayenesinde posterior servikal bölgede ağrılı lenfadenopati ve retrofarengeal apse varlığı saptandı. Hastaya genel anestezi altında cerrahi müdahale yapıldı. Tularemi tanısı serolojik testlerle konuldu. Hasta antimikrobiyal tedavi sonrası sekelsiz olarak taburcu edildi.

We report a 40-year-old woman with characteristic imaging findings for diagnosis of VMCs associated with polycystic kidney disease. Multiple micro-nodular lesions, many of which were cystic, were detected on ultrasound (US). There were echogenic mural nodules and comet tail artifacts in their internal structures. In magnetic resonance imaging (MRI), the mural nodular enhancement and starry sky appearance were observed. The presence of endocystic mural nodules in both US and MRI, as well as the starry sky appearance that shows VMCs, are very important findings for the diagnosis of this condition. Özet Von Meyenburg kompleksleri (VMC'ler), karaciğerin nadir görülen doğuştan, fibropolikistik iyi huylu hastalıklarından biridir. Polikistik böbrek hastalığı ile ilişkili VMC'lerin tanısı için karakteristik görüntüleme bulguları olan 40 yaşında bir kadın olguyu sunuyoruz. Ultrasonografik (US) görüntülemede çoğu kistik yapıda olan çok sayıda mikronodüler lezyon tespit edildi. İç yapılarında ekojenik mural nodüller ve kuyruklu yıldız artefaktları vardı. Manyetik rezonans görüntülemede (MRG), mural nodüler kontrastlanma ve yıldızlı gökyüzü görünümü gözlemlendi. Hem US hem de MRG'de endokistik mural nodüllerin varlığı ve VMC'leri gösteren yıldızlı gökyüzü görünümü bu durumun tanısı için çok önemli bulgulardır.

Despite the substantial developments in oncology, cancer is still not in the category of curative diseases. However, significant steps are being taken in this direction through the identification of factors that play a role in the stages of complex carcinogenesis. Recent data obtained from human gut microbiota has shown that commensal microorganism species living on the epithelial surfaces of the intestines have an active role in this process and are also associated with personal responses to cancer treatment and toxicity. We now know more about the role of Fusobacterium nucleatum, an important member of the gut microbiota complex, as well as other microorganisms in the formation, treatment, diagnosis, and screening of colorectal cancer. In conclusion, the growing knowledge of microbiota, a key factor in colorectal cancer formation and treatment stages, will be able to change the early detection, treatment, and follow-up of these cancers. In this review, the role of gut microbiota in colorectal carcinogenesis and cancer formation mechanisms, significance in treatment and prognosis, and potential use in cancer prevention and screening have evaluated based on the data obtained from recent studies. Özet Bağırsak mikrobiyota disbiyozu inflamatuar bağırsak hastalıkları, nöropsikiyatrik bozukluklar, hipertansiyon, ateroskleroz, hepatit, diyabet, obezite, astım ve kanser gelişimi ile ilişkilendirilmektedir. Onkolojideki önemli gelişmelere rağmen kanser halen tamamen tedavi edilebilir hastalıklar kategorisinde yer almamaktadır. Ancak kompleks karsinogenezin evrelerinde rol oynayan faktörlerin belirlenmesi ile bu yönde önemli adımlar atılmaktadır. İnsan bağırsak mikrobiyotasından elde edilen son veriler, bağırsakların epitelyal yüzeylerinde yaşayan kommensal mikroorganizma türlerinin bu süreçte aktif rol oynadığını ve ayrıca kanser tedavisine ve toksisiteye verilen bireysel yanıtlarla ilişkili olduğunu göstermiştir. Bağırsak mikrobiyota kompleksinin önemli bir üyesi olan Fusobacterium nucleatum'un ve bazı diğer mikroorganizmaların kolorektal kanserin oluşumu, tedavisi, teşhisi ve taranmasındaki rolü hakkında günümüzde daha fazla şey biliyoruz. Sonuç olarak, kolorektal kanser oluşumunda ve tedavi aşamalarında önemli bir faktör olan mikrobiyota konusunda artan bilgi birikimi, bu kanserlerin erken teşhis, tedavi ve takibini değiştirecektir. Bu derlemede, son çalışmalardan elde edilen veriler temelinde, bağırsak mikrobiyotasının kolorektal karsinogenez ve kanser oluşum mekanizmalarındaki rolü, tedavi ve prognozdaki önemi ve kanser önleme ve taramasında kullanım potansiyeli değerlendirilmiştir.

Since management is different, it is important to make a differential diagnosis of these two diseases. Özet Round ligament varisleri, gebelikte nadir görülen bir sağlık sorunu olup klinik bulgular kasık fıtığı ile oldukça benzerlik göstermektedir. Hastalar genellikle kasık ağrısı ve Valsalva manevrası ile büyüyen kasık şişliği şikayetiyle başvurmaktadır. Klinik yönetimi inguinal herniden farklı olduğu için bu iki hastalığın ayırıcı tanısının yapılması önemlidir.

In this article, we aimed to present a case of acute transverse myelitis that developed after laparoscopic total gastrectomy under general anesthesia. Özet Cerrahi müdahaleden sonra akut transvers miyelit nadir görülen bir komplikasyondur. Bu durumun öncelikle spinal veya epidural anestezi ile ilişkili olabileceği düşünülmekte olup genel anesteziden sonra çok nadir görülmektedir. Bu yazıda genel anestezi eşliğinde laparoskopik total gastrektomi sonrası gelişen akut transvers miyelit olgusunu sunmayı amaçladık.

Entecavir, a guanosine nucleoside analog (NA), is a commonly used and generally safe therapeutic agent in HBV infection. In this paper, we aimed to present a case of chronic HBV that developed severe myopathy as an extremely rare side effect after the use of entecavir. Patients receiving entecavir therapy for chronic HBV should be closely monitored for the development of myopathy as well as other known and common side effects. Özet Kronik Hepatit B virusu (HBV) enfeksiyonu, dünya çapında milyonlarca insanı etkileyen ve karaciğer yetmezliği ve hepatoselüler karsinoma gibi ciddi sonuçlara neden olabilen kronik viral bir hastalıktır. Bir guanozin nükleozit analoğu (NA) olan entekavir, HBV enfeksiyonunda yaygın olarak kullanılan ve genellikle güvenli bir terapötik ajandır. Bu yazıda, entekavir kullanımından sonra son derece nadir bir yan etki olarak şiddetli miyopati gelişen bir kronik HBV olgusunu sunmayı amaçladık. Kronik HBV için entekavir tedavisi alan hastalar, miyopati gelişimi ve diğer bilinen ve yaygın yan etkiler açısından yakından izlenmelidir.

Bu tanımlayıcı anket çalışması, Türkiye'de Mart 2021 ve Haziran 2021 tarihleri arasında aile hekimliği polikliniklerinde yetişkinlerle yürütülmüştür. Anket 43 soru içeriyordu; ilk bölümde katılımcılardan bazı sosyodemografik verileri sağlamaları istendi; ikinci bölümde ise mevcut sigara içme durumu, kronik hastalık varlığı, herhangi bir kanser türünün olup olmadığı, ailede kanser öyküsü, ülkesindeki kanser tarama programı hakkında bilgi alma durumu, COVID-19 salgını öncesinde ve sırasında herhangi bir kanser taraması yaptırma öyküsü anketin üçüncü bölümü ise ulusal kanser tarama programı (kolorektal kanser, serviks kanseri, meme kanseri) ile ilgili sorulara odaklandı. Ankete katılan 191 kişiden %54.2'si pandeminin dışkıda gizli kan testi yaptırma sıklığını etkilediğini, %47.4'ü pandeminin kolonoskopi sıklığını etkilediğini, %37.7'si pandeminin mamografi çekme sıklığını etkilediğini ve %37.1'i pandeminin pap smear testi yaptırma sıklığını etkilediğini belirtti. Bu gecikmelerin en çok belirtilen nedeni enfeksiyon kapma endişesiyle sağlık ocağına gitmekten korkmaktı. Kanser taramasındaki kesintiler, mevcut kanserlerin geç teşhisine, kanserle ilişkili ek ölümlere yol açacaktır. Tarama oranları COVID öncesi seviyelere dönene kadar bu kesintilerin büyük bir etkisi olabilir. Sonuçlarımız pandeminin tarama programları üzerine olumsuz etkilerini gösterdi ve en çok vurgulanan neden enfeksiyon kapma endişesiyle sağlık ocağına gitmekten korkmaktı.

Doğada genellikle kuyu suyu, böceklerin bağırsakları ve sürüngenlerde bulunan Y. regensburgei insanlarda septik artrit, yumuşak doku enfeksiyonu ve otitis eksterna gibi farklı tiplerde enfeksiyonlara neden olabilmektedir. Bu yazıda Y. regensburgei’nin neden olduğu ve travmaya bağlı gelişen bir yumuşak doku enfeksiyonu vakası sunulmuştur. Abstract Yokenella regensburgei is a fermentative, motile, oxidase-negative, gram-negative bacillus belonging to the Enterobacteriaceae family and is an opportunistic human pathogen that is caused infections mainly in immunocompromised patients and rarely in immunocompetent individuals. Y. regensburgei, which is generally found in well water, insect guts, and reptiles in nature, can cause different types of infections such as septic arthritis, soft tissue infection, and otitis externa in humans. In this article, a case of soft tissue infection caused by Y. regensburgei and due to trauma is presented.
