Extrinsic causes of restrictive lung syndrome in idiopathic pulmonary fibrosis (IPF) patients have been poorly investigated. We aimed to investigate the influence of the anterior chest wall deformity, noninvasively assessed by modified Haller index (MHI), on spirometry parameters and outcome in a consecutive population of patients with mild-to-moderate IPF. 60 consecutive IPF patients (73.8 ± 6.6 yrs, 45 males) were included in this retrospective study. All patients underwent physical examination, spirometry, blood tests, conventional transthoracic echocardiography and MHI assessment (chest transverse diameter over the distance between sternum and spine) at basal evaluation. During follow-up, we evaluated the composite endpoint of 1) pulmonary or cardiovascular hospitalizations; 2) all-cause mortality. IPF patients with concave-shaped chest wall (MHI >2.5) (36.7% of total) and those with normal chest shape (MHI ≤2.5) (63.3%) were separetely analyzed. In comparison to IPF patients with MHI ≤2.5, those with MHI >2.5: were less likely to be men and smokers; had a more severe restrictive pattern; had significantly smaller cardiac chamber dimensions and significantly higher systolic pulmonary artery pressure (51.9±15.1 vs 42.4±14.3 mmHg, p=0.02). Mean follow-up time was 2.5±1.4 yrs. During follow-up, 13 deaths and 16 pulmonary or cardiovascular hospitalizations were detected. At multivariate Cox regression analysis, concave-shaped chest wall (MHI>2.5) (HR 4.55, 95%CI 1.02-20.4), increased C-reactive protein (HR 1.68, 95%CI 1.08-2.61) and absence of beta-blocker therapy (HR 0.13, 95%CI 0.01-0.26) were independently associated to the investigated outcome. MHI assessment and implementation may help the clinician to identify, among IPF patients, those with poorer prognosis over a medium-term follow-up.

Publisher: Radboud Data RepositoryCountry: NetherlandsAlpha oscillations (8���14 Hz) are the dominant rhythm in the awake brain, which is proposed to set the internal state of the neural system. Previous work has demonstrated that states of decreased spontaneous alpha oscillatory power correlate with enhanced neural excitability. However, whether and how changes in neural excitability influence sensory processing and manifest in behavior are yet open questions. To address these questions, we used a novel paradigm to experimentally manipulate human participants��� decision criterion (i.e., how certain percepts are used for decision) in a visual detection task. In different blocks, we primed participants differently with clearly visible stimuli to introduce either a liberal or conservative decision criterion for the subsequently presented ambiguous stimuli. While we observed prevalent criterion shifts in participants��� decisions under different priming conditions, the pre-stimulus oscillatory power or phase were not modulated by different priming conditions. Trial-by-trial analyses showed that lower spontaneous alpha oscillatory power in visual areas encoding the target stimuli leads to higher accuracy in performing the task, which is accompanied with enhanced information content decodable in the neural activity patterns. Moreover, spontaneous alpha oscillatory phase in anterior frontal and superior parietal regions modulate accuracy in a phasic sinusoidal manner. Together, our results indicate that as neural excitability increases in the state of decreased occipital alpha-band oscillatory power, the sensory representation of task-relevant visual stimuli is sharpened and consequently leads to improved behavioral discriminability.

it onderzoek was noodzakelijk gebleken, na eerder uitgevoerd vooronderzoek. Uit het Bureauonderzoek is gebleken dat het plangebied een hoge kans heeft op het aantreffen van archeologische waarden. Tijdens het veldwerk is vervolgens gebleken dat de bodemopbouw grotendeels intact is. Verspreid over het plangebied zijn intacte Podzolprofielen waargenomen. Van de 29 gezette boringen vertoonden slechts 4 boringen een duidelijk verstoorde bodemopbouw. Er zijn echter geen archeologische indicatoren waargenomen. Het niet aantreffen van archeologische indicatoren hoeft echter niet te betekenen dat er geen archeologische waarden aanwezig zijn. Aangezien het moeilijk is alle archeologische vindplaatsen in zandgebieden met behulp van boringen aan te tonen, dient de conclusie dan ook te zijn dat, ondanks het ontbreken van archeologische indicatoren, de kans groot is dat zich binnen het plangebied archeologische waarden bevinden.1 Het nu uitgevoerde proefsleuvenonderzoek heeft bevestigd dat de bodemopbouw inderdaad grotendeels intact is zoals was vastgesteld bij het booronderzoek. De eerste 30 cm bestaat uit humeuze, fijnzandige bouwvoor (Ap horizont). De bouwvoor gaat over in een ongeploegde humeuze fijnzandige laag, de enkeerdgronden (Aan horizont). De dikte van de enkeerdgronden varieert over het plangebied van 30 tot 115 cm, maar bedraagt gemiddeld 85 cm. Op een diepte van gemiddeld 115 cm –mv is een grijze tot lichtgrijze laag aangetroffen, de uitspoelingshorizont (E horizont). Deze laag is ontstaan doordat in de loop van de tijd de humus is uitgespoeld. De dikte van de uitspoelingshorizont bedraagt gemiddeld 10 cm. Onder de E- horizont is een donkere, humeuze en ijzerhoudende inspoelingshorizont gevormd (B horizont). De dikte van deze laag bedraagt gemiddeld 20-30 cm. De B-horizont gaat vervolgens over in lichtgeel, licht leemhoudend fijn zand, de C-horizont. Bij het onderzoek zijn archeologische waarden aangetroffen. Het betreffen grondsporen en vondsten die behoren bij een nederzetting uit de middeleeuwen, 11 e-15e eeuw. De grondsporen bestaan uit paalgaten welke toe te schrijven zijn aan een boerderij van het Dommelentype alsook paalgaten van bijgebouwen. Tevens zijn er greppels vastgesteld die waarschijnlijk onderdeel zijn van een erfafscheiding en kuilen die voor water of afval gediend kunnen hebben. Tijdens het proefsleuvenonderzoek zijn zes vondsten gedaan. Tijdens de aanleg van de vlakken zijn in een onverstoorde es twee wandscherven aardewerk uit de 15e – 17e eeuw aangetroffen. Deze zijn waarschijnlijk bij de bemesting van de akker daar terecht gekomen. De rest van de vondsten komt uit antropogene sporen van de Middeleeuwse nederzetting. Het handelt om een wandscherf grijs aardewerk, een soort die vanaf de 11e tot 15e eeuw is geproduceerd, een vuursteenafslag zonder bewerkingssporen, een stuk verbrande klei dat bouwmateriaal zou kunnen zijn en een stuk natuursteen

it onderzoek was noodzakelijk gebleken, na eerder uitgevoerd vooronderzoek. Uit het Bureauonderzoek is gebleken dat het plangebied een hoge kans heeft op het aantreffen van archeologische waarden. Tijdens het veldwerk is vervolgens gebleken dat de bodemopbouw grotendeels intact is. Verspreid over het plangebied zijn intacte Podzolprofielen waargenomen. Van de 29 gezette boringen vertoonden slechts 4 boringen een duidelijk verstoorde bodemopbouw. Er zijn echter geen archeologische indicatoren waargenomen. Het niet aantreffen van archeologische indicatoren hoeft echter niet te betekenen dat er geen archeologische waarden aanwezig zijn. Aangezien het moeilijk is alle archeologische vindplaatsen in zandgebieden met behulp van boringen aan te tonen, dient de conclusie dan ook te zijn dat, ondanks het ontbreken van archeologische indicatoren, de kans groot is dat zich binnen het plangebied archeologische waarden bevinden.1 Het nu uitgevoerde proefsleuvenonderzoek heeft bevestigd dat de bodemopbouw inderdaad grotendeels intact is zoals was vastgesteld bij het booronderzoek. De eerste 30 cm bestaat uit humeuze, fijnzandige bouwvoor (Ap horizont). De bouwvoor gaat over in een ongeploegde humeuze fijnzandige laag, de enkeerdgronden (Aan horizont). De dikte van de enkeerdgronden varieert over het plangebied van 30 tot 115 cm, maar bedraagt gemiddeld 85 cm. Op een diepte van gemiddeld 115 cm –mv is een grijze tot lichtgrijze laag aangetroffen, de uitspoelingshorizont (E horizont). Deze laag is ontstaan doordat in de loop van de tijd de humus is uitgespoeld. De dikte van de uitspoelingshorizont bedraagt gemiddeld 10 cm. Onder de E- horizont is een donkere, humeuze en ijzerhoudende inspoelingshorizont gevormd (B horizont). De dikte van deze laag bedraagt gemiddeld 20-30 cm. De B-horizont gaat vervolgens over in lichtgeel, licht leemhoudend fijn zand, de C-horizont. Bij het onderzoek zijn archeologische waarden aangetroffen. Het betreffen grondsporen en vondsten die behoren bij een nederzetting uit de middeleeuwen, 11 e-15e eeuw. De grondsporen bestaan uit paalgaten welke toe te schrijven zijn aan een boerderij van het Dommelentype alsook paalgaten van bijgebouwen. Tevens zijn er greppels vastgesteld die waarschijnlijk onderdeel zijn van een erfafscheiding en kuilen die voor water of afval gediend kunnen hebben. Tijdens het proefsleuvenonderzoek zijn zes vondsten gedaan. Tijdens de aanleg van de vlakken zijn in een onverstoorde es twee wandscherven aardewerk uit de 15e – 17e eeuw aangetroffen. Deze zijn waarschijnlijk bij de bemesting van de akker daar terecht gekomen. De rest van de vondsten komt uit antropogene sporen van de Middeleeuwse nederzetting. Het handelt om een wandscherf grijs aardewerk, een soort die vanaf de 11e tot 15e eeuw is geproduceerd, een vuursteenafslag zonder bewerkingssporen, een stuk verbrande klei dat bouwmateriaal zou kunnen zijn en een stuk natuursteen
