    ABSTRACT Objective To identify the effect of social capital on adolescent smoking. Method A stratified random sample of 1313 7th and 8th grade students from three counties in Transylvania, Romania, completed a self-administered questionnaire on smoking-related knowledge, attitudes and behaviours. The impact of social capital was measured (personal and community activities, school achievements and smoking-related knowledge). Multivariate multinomial logistic regression models were used to measure the association between social participation and smoking. Results Experimenting with smoking was mostly related to knowledge about smoking, academic performance and second-hand tobacco smoke exposure at home. The strongest risk factor of adolescent smoking was the smoking behaviour of classmates: those who reported a significant proportion of smokers among their classmates were nine times more likely to smoke themselves than in other cases (adjusted odds ratio [aOR]: 9.05). Those who considered smoking to be harmless were 4 times more likely to be smokers than those who considered this behaviour to be dangerous (aOR: 4.28). Poor academic results increased adolescents’ smoking (aOR: 3.22 and 2.66). The odds were significantly higher for smoking, if they had an active social life (aOR: 2.54). Regular church attendance proved to be a protective factor (aOR: 0.45). Conclusions Several social capital factors can play a role in adolescent smoking. The organization and the development of community activities aimed at prevention must strengthen the factors related to the community's social capital to reduce the likelihood of teenage smoking. RESUMEN Objetivo Evaluar el efecto del capital social sobre el consumo de tabaco en adolescentes. Método El estudio se realizó en un grupo aleatorizado y estratificado compuesto por 1313 estudiantes de séptimo y octavo grado de tres municipios en Transilvania (Rumanía). Los participantes contestaron un cuestionario autocumplimentado en relación con el consumo de tabaco y sobre actitudes y comportamientos respecto a este. El impacto del capital social, entendido como actividades personales y comunitarias, así como el desarrollo académico y la información sobre el consumo de tabaco, fueron algunas de las medidas. Resultados Experimentar con el hábito de fumar se relaciona principalmente con el conocimiento sobre dicha adicción, el rendimiento académico y la exposición al consumo de tabaco en el hogar. El mayor riesgo para el consumo se deriva de la interacción con compañeros de clase que incurren en el consumo de tabaco. En tal caso, la probabilidad de consumo aumenta nueve veces (odds ratio ajustada [ORa]: 9,05). Quienes consideraron que fumar es inofensivo tuvieron cuatro veces más probabilidades de ser fumadores que quienes consideraron este comportamiento como peligroso (ORa: 4,28). En cuanto a los/las estudiantes con bajo rendimiento académico se observa un mayor incremento del consumo (ORa: 3,22 y 2,66). Al mismo tiempo, dicho patrón también se observa entre aquellos/as con un entorno social activo (ORa: 2,54). La asistencia a la iglesia de manera regular es un factor protector (ORa: 0,45). Conclusiones Diferentes aspectos relacionados con el capital social se asocian al consumo de tabaco en adolescentes. La organización de actividades y el desarrollo comunitario deberán tener en cuenta estos aspectos para prevenir el consumo de tabaco. Hay que prestar especial atención a la clase social con el fin de reducir las probabilidades de consumo en adolescentes.

    BackgroundHypergastrinaemia occasionally indicates the presence of a gastrinoma. However it is much more commonly associated with various benign causes including proton pump inhibitor (PPI) use, Helicobacter pylori infection and/or atrophic gastritis. The extent to which these factors interact to influence fasting serum gastrin concentrations remains incompletely understood.Materials and MethodsFasting serum gastrin concentrations were measured by radioimmunoassay in 1,400 patients attending for diagnostic oesophagogastro-duodenoscopy. After exclusions, 982 patients were divided into four groups and their results analysed. We compared gastrin concentrations in normal patients (no H. pylori infection, no PPI use and no histological evidence of gastric preneoplasia (n=233)), with those in patients who were taking regular PPIs (H. pylori negative with no gastric preneoplasia (n=301)), patients who had active H. pylori infection but no gastric preneoplasia (n=164) and patients with histologically confirmed gastric preneoplasia (n=284).ResultsMedian fasting gastrin concentration in the normal group was 20pM and was significantly increased in PPI users (46pM, p<0.0001), patients with active H. pylori infection (27pM, p<0.0001), and patients with antral (25pM, p<0.01) or corpus (48pM, p<0.0001) gastric preneoplasia. PPI use resulted in further significant increases in fasting serum gastrin concentrations in patients who were infected with H. pylori (50pM, n=56) or who had antral gastric preneoplasia (53pM, n=87), but did not significantly alter serum gastrin concentrations in patients with corpus preneoplasia (90pM, n=66).ConclusionsPPI use, H. pylori infection and atrophic gastritis all caused significant elevations of median fasting gastrin concentrations. However, several patients who had potential risk factors for hypergastrinaemia still demonstrated fasting serum gastrin concentrations within the normal range.

    Absztrakt: A szerzők egy negatív családi anamnézisű, súlyos, neonatalis kezdetű epilepsziás encephalopathiában szenvedő, jelenleg kétéves fiúgyermek esetét mutatják be. A terápiarezisztens epilepszia és az igen súlyos fejlődésbeli elmaradás etiológiáját kiterjedt klinikai vizsgálatokkal sem sikerült tisztázni. Genetikai okot feltételezve, külföldi genetikai laboratóriumban 128 gént tartalmazó újgenerációs szekvenálási (NGS-) panelvizsgálatot indikáltak epilepsziás encephalopathiát okozó betegségek irányában. A vizsgálat egy eddig ismeretlen hemizigóta misszensz mutációt igazolt a MECP2-génben. A szerzők az esetbemutatás kapcsán áttekintik a lányokban klasszikusan Rett-szindrómát okozó, a MECP2-gén mutációi által előidézett idegfejlődési rendellenességek spektrumát fiúkban. Más, X-hez kötött domináns öröklődésű betegségekhez hasonlóan sokáig úgy gondolták, hogy a MECP2-gén-mutációt hordozó fiúmagzatok életképtelenek, napjainkra azonban ez a nézet megváltozott. A szerzők úgy tudják, hogy betegük az első magyar fiúgyermek, akinél a MECP2-gén mutációja igazolódott. Orv Hetil. 2019; 160(51): 2036–2039.

    Abstract: Introduction: There is a high mortality with not well understood risk factors after the second hip fracture. Aim: Analysis of the 30- and 365-day mortality and its risk factors in patients with contralateral hip fracture. Method: Patients with contralateral hip fracture between 01 Jan 2000 and 31 Dec 2008 were identified among those who suffered their primary hip fracture in Hungary in 2000. Risk factors as age, sex, concomitant and chronic diseases, type of fracture and surgery, surgical complications, day of admission were analyzed by logistic and Cox regression as well as Kaplan-Meier analysis. Results: There were 312 eligible patients identified with 8.3 % mortality rate at 30 and with 38,4% at 365 days respectively. Significant risk factors for the 30 day mortality were intertrochanteric type of fracture (OR: 4.722; HR: 4.129) and non operative management (OR: 7.357; HR: 6.317) while for the 365 day mortality those were older age (OR:1.070; HR:1.050) and type of surgery (OR: 0.450). Conclusion: Age, type of fracture and type of surgery proved to be risk factors.     Yuriy Shumovsky is a famous scientist, ethnographer, archaeologist, and priest who made a significant contribution to the material culture of his homeland. The urgency of the research topic lies in the fact that today’s existing knowledge does not provide full information about the foundation of museology by Yuriy Shumovsky in the Rivne region during the occupation period of 1941–1944. In the modern historiographical science, despite a significant number of publications, there are no scientific studies that would fully summarize and evaluate the importance of Yuriy Shumovsky’s hard work as the main founder of the museology in the Rivne region of this period. The purpose of the study is to disclose fully the participation of Yuri Shumovsky in organizing a museum in Rivne region during the German occupation (1941–1944). The archival documents and memoirs of the scientist concerning the activity of the Rivne Museum of Local History are presented in the article. The methodological basis of the research is the principle of historicism, systematicity and objectivity. The problem-chronological approach is applied while presenting the material. Particular attention was paid to the methodology of working with archival sources. According to the results received after the examination of the source base, a general picture of the functioning of the museum during the occupation period has been reproduced. The cultural-educational and research activity of the museum has been discovered. It included conducting regional studies and researches on the territory of the region and archaeological and ethnographic studies; replenishing museum collections by valuable findings; organizing educational activities; promoting the preservation and protection of the historical monuments, publishing scientific-popular works. Moreover, the staffing structure, financial support and budget of the museum have been examined and discussed. The article also mentions the availability of the museum inventory that has been found which is an important source for restoring information about the number of exhibits of five departments and the extent of the loss of museum valuables. The importance of Yuriy Shumovsky’s persistent work as the main founder of museology in the Rivne region of this period is determined. Yuriy Shumovsky's museological work in a particular collection, classification, conservation and description of exhibits has been also assessed.     This article discusses the relationship of men towards Istrian cattle ( boskarin ) through the years. The authors question the problem of treating the animals as objects of natural-cultural heritage. The concepts of Nature and Culture remain anthropocentrically segregated, and this is also reflected in promoting the boskarin as a gastro-phenomenon, which was the dominant feature of the project Boskarin with Potatoes (2012–2014) by the City of Pula, developed as the collaboration with the French town of Villefranche-de-Rouergue, which became the partner city of Pula in 2008. The authors claim that one can only talk about heritage species (especially traditional or autochthonous livestock breeds), but not about heritage animals (the animals in the concept of heritage), and that the anthropology of animals, as defined for instance by socio-cultural anthropologist Barbara Noske, can be – or ought to be – a segment of ethnographic studies of the 21 st century.

    Keratinocytes are one of the primary cells affected by psoriasis inflammation. Our study aimed to delve deeper into their morphology, transcriptome, and epigenome changes in response to psoriasis-like inflammation. We created a novel cytokine mixture to mimic mild and severe psoriasis-like inflammatory conditions in cultured keratinocytes. Upon induction of inflammation, we observed that the keratinocytes exhibited a mesenchymal-like phenotype, further confirmed by increased VIM mRNA expression and results obtained from confocal microscopy. We performed RNA sequencing to achieve a more global view, revealing 858 and 6987 DEGs in mildly and severely inflamed keratinocytes, respectively. Surprisingly, we found that the transcriptome of mildly inflamed keratinocytes more closely mimicked that of the psoriatic epidermis transcriptome than the severely inflamed keratinocytes. Genes involved in the IL-17 pathway were a major contributor to the similarities of the transcriptomes between mildly inflamed KCs and psoriatic epidermis. Mild and severe inflammation led to the gene regulation of epigenetic modifiers such as HATs, HDACs, DNMTs, and TETs. Immunofluorescence staining revealed distinct 5-hmC patterns in inflamed versus control keratinocytes, and consistently low 5-mC intensity in both groups. However, the global DNA methylation assay detected a tendency of decreased 5-mC levels in inflamed keratinocytes versus controls. This study emphasizes how inflammation severity affects the transcriptomic similarity of keratinocytes to psoriatic epidermis and proves dynamic epigenetic regulation and adaptive morphological changes in inflamed keratinocytes.

Most már második éve általános tapasztalat – és nemcsak egy kis területen, hanem az egész világon – a COVID–19 halálos járvány terjedése (az elnevezés pontosan: SARS-CoV-2), amelyet már nem lehetett egyszerű megfázásnak vagy gyorsan elmúló betegségnek tekinteni, hanem egy olyan súlyos állapotnak, mely fenyegeti minden ember életét, függetlenül attól, hogy ki hol él. Az egészségügyi kutatók az ellenanyag kifejlesztéséért küzdöttek, mely megállíthatja a vírus terjedését. A gyógyítás kérdései mellett azonban filozófiai és vallási kérdések is felmerültek, melyekre pszichológusok és hittudósok is keresik a választ. Az ember teremtettségéből és szabad akaratából kiindulva a jelen tanulmány a Bibliában közölt kinyilatkoztatás és a keresztény tanítás alapján vállalkozik nemcsak lehetséges válaszokat adni, hanem a járvány komolyságával számolva a jelenség hitbeli és teológiai magyarázatát is megfogalmazni. Summary. The Covid-19 epidemic, which began in late 2019 and early 2020, has reached everyone: the family, the workplace, and public life. The phenomenon also requires a comprehensive solution. The unfortunate experience has taught everyone that there is no age limit for the virus because it affects everyone equally. It has become clear that it is not individual solutions that are needed, but only community path finding, that no one lives alone in isolation. Throughout its two-thousand-year history, Christianity has accompanied man in both its successes and failures. God’s revelation-based teaching is unchanged and to this day He provides His resulting answers to everyone by placing the whole man before God. This Christian-minded anthropology means that man lives in a personal relationship that assigns him to God alone. In terms of faith, it thus approaches the crisis, the disease, the drama of being, in a different way, which sees the reality of life and death in a greater context. The spread of the current epidemic has been interpreted by many as a divine punishment. But God, the Creator, who is good, cannot be the source of good and evil at the same time, he cannot be the starting point of evil. It is precisely because of these characteristics that spiritual and moral behavior is valued in the epidemiological situation. Christianity does not see God-consciously directing punitive action in unpleasant events for people and humanity, as God is first and foremost a Father. In the Old Testament, there is no connection between a particular disease or other plague and the personal sins of those who suffer it. Far from the New Testament, the conclusion is that it links the plagues, diseases, and epidemics we experience to the sins committed by individual people. The turn of the New Testament, then, is also significant in the presentation of God. It is no longer the image of the fearsome and terrible God that emerges, but the teaching of Jesus, who says of His Father, “God is love.” The God of the New Testament is a God of co-suffering and compassion who thinks not of punishment but of forgiveness. Troubles are not necessarily attributable to the sins of individuals, but actually to the sins of all mankind. Therefore, it is necessary for everyone to feel somehow responsible for the other when they see that their sins continue. However, the viral situation has also shown unparalleled human behaviors and values, especially on the part of healthcare professionals. Perhaps involuntarily, among those who acted for others, man’s better self manifested itself, so they were able to make sacrifices as well. Of them, too, Jesus said, just before his suffering, that “no one loves more than he who gives his life for his friends.” It is love that overcomes fear, that can be the cure for a virus.

    ABSTRACT Objective To identify the effect of social capital on adolescent smoking. Method A stratified random sample of 1313 7th and 8th grade students from three counties in Transylvania, Romania, completed a self-administered questionnaire on smoking-related knowledge, attitudes and behaviours. The impact of social capital was measured (personal and community activities, school achievements and smoking-related knowledge). Multivariate multinomial logistic regression models were used to measure the association between social participation and smoking. Results Experimenting with smoking was mostly related to knowledge about smoking, academic performance and second-hand tobacco smoke exposure at home. The strongest risk factor of adolescent smoking was the smoking behaviour of classmates: those who reported a significant proportion of smokers among their classmates were nine times more likely to smoke themselves than in other cases (adjusted odds ratio [aOR]: 9.05). Those who considered smoking to be harmless were 4 times more likely to be smokers than those who considered this behaviour to be dangerous (aOR: 4.28). Poor academic results increased adolescents’ smoking (aOR: 3.22 and 2.66). The odds were significantly higher for smoking, if they had an active social life (aOR: 2.54). Regular church attendance proved to be a protective factor (aOR: 0.45). Conclusions Several social capital factors can play a role in adolescent smoking. The organization and the development of community activities aimed at prevention must strengthen the factors related to the community's social capital to reduce the likelihood of teenage smoking. RESUMEN Objetivo Evaluar el efecto del capital social sobre el consumo de tabaco en adolescentes. Método El estudio se realizó en un grupo aleatorizado y estratificado compuesto por 1313 estudiantes de séptimo y octavo grado de tres municipios en Transilvania (Rumanía). Los participantes contestaron un cuestionario autocumplimentado en relación con el consumo de tabaco y sobre actitudes y comportamientos respecto a este. El impacto del capital social, entendido como actividades personales y comunitarias, así como el desarrollo académico y la información sobre el consumo de tabaco, fueron algunas de las medidas. Resultados Experimentar con el hábito de fumar se relaciona principalmente con el conocimiento sobre dicha adicción, el rendimiento académico y la exposición al consumo de tabaco en el hogar. El mayor riesgo para el consumo se deriva de la interacción con compañeros de clase que incurren en el consumo de tabaco. En tal caso, la probabilidad de consumo aumenta nueve veces (odds ratio ajustada [ORa]: 9,05). Quienes consideraron que fumar es inofensivo tuvieron cuatro veces más probabilidades de ser fumadores que quienes consideraron este comportamiento como peligroso (ORa: 4,28). En cuanto a los/las estudiantes con bajo rendimiento académico se observa un mayor incremento del consumo (ORa: 3,22 y 2,66). Al mismo tiempo, dicho patrón también se observa entre aquellos/as con un entorno social activo (ORa: 2,54). La asistencia a la iglesia de manera regular es un factor protector (ORa: 0,45). Conclusiones Diferentes aspectos relacionados con el capital social se asocian al consumo de tabaco en adolescentes. La organización de actividades y el desarrollo comunitario deberán tener en cuenta estos aspectos para prevenir el consumo de tabaco. Hay que prestar especial atención a la clase social con el fin de reducir las probabilidades de consumo en adolescentes.

    BackgroundHypergastrinaemia occasionally indicates the presence of a gastrinoma. However it is much more commonly associated with various benign causes including proton pump inhibitor (PPI) use, Helicobacter pylori infection and/or atrophic gastritis. The extent to which these factors interact to influence fasting serum gastrin concentrations remains incompletely understood.Materials and MethodsFasting serum gastrin concentrations were measured by radioimmunoassay in 1,400 patients attending for diagnostic oesophagogastro-duodenoscopy. After exclusions, 982 patients were divided into four groups and their results analysed. We compared gastrin concentrations in normal patients (no H. pylori infection, no PPI use and no histological evidence of gastric preneoplasia (n=233)), with those in patients who were taking regular PPIs (H. pylori negative with no gastric preneoplasia (n=301)), patients who had active H. pylori infection but no gastric preneoplasia (n=164) and patients with histologically confirmed gastric preneoplasia (n=284).ResultsMedian fasting gastrin concentration in the normal group was 20pM and was significantly increased in PPI users (46pM, p<0.0001), patients with active H. pylori infection (27pM, p<0.0001), and patients with antral (25pM, p<0.01) or corpus (48pM, p<0.0001) gastric preneoplasia. PPI use resulted in further significant increases in fasting serum gastrin concentrations in patients who were infected with H. pylori (50pM, n=56) or who had antral gastric preneoplasia (53pM, n=87), but did not significantly alter serum gastrin concentrations in patients with corpus preneoplasia (90pM, n=66).ConclusionsPPI use, H. pylori infection and atrophic gastritis all caused significant elevations of median fasting gastrin concentrations. However, several patients who had potential risk factors for hypergastrinaemia still demonstrated fasting serum gastrin concentrations within the normal range.

    Absztrakt: A szerzők egy negatív családi anamnézisű, súlyos, neonatalis kezdetű epilepsziás encephalopathiában szenvedő, jelenleg kétéves fiúgyermek esetét mutatják be. A terápiarezisztens epilepszia és az igen súlyos fejlődésbeli elmaradás etiológiáját kiterjedt klinikai vizsgálatokkal sem sikerült tisztázni. Genetikai okot feltételezve, külföldi genetikai laboratóriumban 128 gént tartalmazó újgenerációs szekvenálási (NGS-) panelvizsgálatot indikáltak epilepsziás encephalopathiát okozó betegségek irányában. A vizsgálat egy eddig ismeretlen hemizigóta misszensz mutációt igazolt a MECP2-génben. A szerzők az esetbemutatás kapcsán áttekintik a lányokban klasszikusan Rett-szindrómát okozó, a MECP2-gén mutációi által előidézett idegfejlődési rendellenességek spektrumát fiúkban. Más, X-hez kötött domináns öröklődésű betegségekhez hasonlóan sokáig úgy gondolták, hogy a MECP2-gén-mutációt hordozó fiúmagzatok életképtelenek, napjainkra azonban ez a nézet megváltozott. A szerzők úgy tudják, hogy betegük az első magyar fiúgyermek, akinél a MECP2-gén mutációja igazolódott. Orv Hetil. 2019; 160(51): 2036–2039.
