    The term "Medical devices" includes technology-based devices or articles, both basic and complex. Due to these types of variations, a strict, robust, transparent, and sustainable regulatory framework is required. In recent clinical practice, incidents including the breast implant and the hip replacement crisis have made it necessary to improve the regulatory and compliance approaches for the industry to ensure the manufacturing and distribution of safe and innovative MDs within the EU. In response to this, the EU revised the laws governing medical devices and in vitro diagnostics to align with the developments of the sector, address critical safety issues and support innovation. The new regulation (EU) 2017/745 on Medical Devices (MDR) is now applicable from May 26 2021 and the In Vitro Diagnostic Medical Devices Regulation (EU) 2017/746 will take effect from May 2022.In this review, we aim to provide an update on the new Medical Device Regulations in the context of the current medical needs of the world, and also to give a glimpse at the non-EU regulatory landscape. Finally, we take a look at the closed-system transfer devices (CSTD) and COVID facilitated changes promoting demand for continuous improvement and trends in the pharmaceutical and medical industry related areas.

    Célunk az volt, hogy az SZTE Belgyógyászati Klinika Nyugati telephely fekvőbeteg Járványellátó Központ részlegén felvételre került, 18 év feletti Covid-pozitív betegek között a kezelt hypertonia előfordulását, a szedett vérnyomáscsökkentők számát, a RAS-gátló vérnyomáscsökkentők (ACE-gátló és/vagy ARB) előfordulását felmérjük. A vizsgálatban véletlenszerűen kiválasztott 165 (94 férfi, 71 nő) gyorsteszttel vagy PCR-rel Covid-pozitív, 2021. február közepe és április vége között felvett betegek adatainak retrospektív elemzése történt. A betegek életkora 59,9±12,9 év volt (26–92), a BMI 30,7±6,6 kg/m2 (20,3–55,4). A betegek között felvételkor 103 (62,4%) már ismert hypertoniás egyén volt (56 férfi, 47 nő). A szedett vérnyomáscsökkentők száma felvételkor 1,6±1,6 darab (maximum hat), a kezelés végén 1,8±1,7 darab. Területen több mint három szert 23-an, éppen hármat 25-en szedtek. Monoterápiában felvételt megelőzően 20-an részesültek, a kezelés végén 25-en. A vérnyomáscsökkentők száma a kezelés végén 74 esetben nem változott, hat esetben csökkent, 31 esetben nőtt, ezek között 11 új indítás volt. Egy betegnél a csökkenés 100%-os volt, a monoterápiát el kellett hagyni, a többi öt esetben pedig 50%-os. A hypertoniás betegek több napot töltöttek intézetben, mint a nem hypertoniás egyének (10,9±6,8 vs. 9,2±6,3). A felvételkor mért spontán O2-szaturáció a hypertoniás beteg között alacsonyabb volt, mint a nem hypertoniás egyéneknél (92,6±4,8 vs. 89,6±9,7%), ARB-t szedők között volt a legalacsonyabb (87,5±8,5%, p=0,01 vs. nem hypertoniás). Az összes halálozás a hypertoniás betegek között 8,7%, a nem hypertoniás betegek között csak 1,6% volt. A hypertoniás betegeket vizsgálva, a RAS-gátlót szedők és nem szedők között az intenzív ellátás 16,3 vs. 13%. Az összes halálozás a RAS-gátlót szedők között 10%, a nem szedők között 4,3%, az ARB-t szedők között 13% és az ACE-gátlót szedők között 8,8% volt. Eredményeink alapján a hypertonia nagyobb kockázatot jelenthet a rosszabb Covid-kimenetel tekintetében. A RAS-gátló kezelésben részesültek nagyobb kockázatát észleltük a RAS-gátló kezelésben nem részesültekhez képest.

    A Covid-19-járvánnyal járó problémák a krónikusbeteg-populációkra vannak a legrosszabb hatással. A stressz, a mindennapi rutin felbomlása, családi problémák, a megbetegedés és az ellátás nehezítettsége miatt a krónikus fájdalommal élők az egyik kiemelkedően veszélyeztetett csoport. Ezért elengedhetetlen felmérni ennek a csoportnak a helyzetét, pszichés jóllétét. A jelen kérdőíves felmérés célja a következő volt: a krónikus fájdalommal élő személyek felmérése a Covid-19-pan­dé­mia alatt, kitérve a fájdalommal összefüggésbe hozható pszichés háttértényezőkre, mint a depresszió, az érzelemszabályozás, az alexithymia, a jóllét, az egészségértés és a társas támogatás. A felmérés 158 főt vizsgált, akiknél legalább 3 hónapja állt fenn valamilyen fájdalom, de orvosi ellátásban nem részesültek. Az adatfelvétel két időpontban zaj­lott: 2021. februárban és decemberben. A pandémiára való tekintettel a felmérés online, kérdőíves módszerrel zajlott. A felmérés során a következő hat pszichológiai tesztet alkalmaztuk: Torontói Alexithymia Skála, Beck Dep­resszió Kérdőív rövidített változata, Érzelemszabályozási Nehézségek Kérdőív, Multidimenzionális Észlelt Társas Támogatás Kérdőív, az egészségértést mérő Chew-kér­dé­sek és a WHO Jóllét Kérdőív. A vizsgálati személyek életkora 20–80 év közötti, 140 (88%) nő. Negyvenkét fő (27%) érte el a súlyos alexithymia mértékét. Depresszió 118 főnél (75%) volt kimutatható, melyből 72 fő (46%) enyhe, 26 fő (16%) közepesen súlyos és 20 fő (13%) súlyos depresszióval érintett. A fájdalom mértéke és az alexithymia [r(158) = 0,16, p = 0,004], a depresszió [r(158) = 0,41, p < 0,001], az érzelemszabályozási nehézségek [r(158) = 0,26, p = 0,004], az egészségértési és érzelemszabályozási nehézségek [r(158) = 0,25, p = 0,001] között pozitív, szignifikáns korreláció volt kimutatható. A fájdalommal élő személyeket jellemző komorbiditások (szorongás, hangulatzavar, alvászavar) mellett a járvány indukálta hosszan tartó szociális izoláció, stressz és a félelem magyarázhatja a depresszió, az ér­ze­lemszabályozási nehézségek és az egészségértési problémák magas arányát a vizsgált mintában, amik tovább súlyosbítják az alexithymia és a fájdalom mértékét. Ezek alapján fontos felhívni a szakemberek figyelmét az érintett személyek megfelelő egészségügyi ellátására és edukációs szükségleteire.

    The pandemic has affected social relations in many ways, and even created new social groups through vaccination. The goal of this study was to explore whether intergroup discrimination regarding vaccination status can be observed in a resource allocation task. Participants (N = 818; Mage = 46.0 years) completed a resource allocation task. Results showed that the better-than-average effect was widespread among the participants as most of them perceived themselves more informed about the pandemic than others. The resource allocation task showed participants preferred to create maximum difference in favor of their group in intergroup situations, but decisions were fairer when targets' group membership was identical. Moreover, vaccinated people were more likely to use maximum difference strategies than non-vaccinated people. The results revealed that vaccination status changes the perception of intergroup situations, which may be important in planning future strategies to handle mass emergencies similar to the current pandemic.

The new regulation (EU) 2017/745 on Medical Devices (MDR) is now applicable from May 26 2021 and the In Vitro Diagnostic Medical Devices Regulation (EU) 2017/746 will take effect from May 2022.In this review, we aim to provide an update on the new Medical Device Regulations in the context of the current medical needs of the world, and also to give a glimpse at the non-EU regulatory landscape. Finally, we take a look at the closed-system transfer devices (CSTD) and COVID facilitated changes promoting demand for continuous improvement and trends in the pharmaceutical and medical industry related areas.

    Célunk az volt, hogy az SZTE Belgyógyászati Klinika Nyugati telephely fekvőbeteg Járványellátó Központ részlegén felvételre került, 18 év feletti Covid-pozitív betegek között a kezelt hypertonia előfordulását, a szedett vérnyomáscsökkentők számát, a RAS-gátló vérnyomáscsökkentők (ACE-gátló és/vagy ARB) előfordulását felmérjük. A vizsgálatban véletlenszerűen kiválasztott 165 (94 férfi, 71 nő) gyorsteszttel vagy PCR-rel Covid-pozitív, 2021. február közepe és április vége között felvett betegek adatainak retrospektív elemzése történt. A betegek életkora 59,9±12,9 év volt (26–92), a BMI 30,7±6,6 kg/m2 (20,3–55,4). A betegek között felvételkor 103 (62,4%) már ismert hypertoniás egyén volt (56 férfi, 47 nő). A szedett vérnyomáscsökkentők száma felvételkor 1,6±1,6 darab (maximum hat), a kezelés végén 1,8±1,7 darab. Területen több mint három szert 23-an, éppen hármat 25-en szedtek. Monoterápiában felvételt megelőzően 20-an részesültek, a kezelés végén 25-en. A vérnyomáscsökkentők száma a kezelés végén 74 esetben nem változott, hat esetben csökkent, 31 esetben nőtt, ezek között 11 új indítás volt. Egy betegnél a csökkenés 100%-os volt, a monoterápiát el kellett hagyni, a többi öt esetben pedig 50%-os. A hypertoniás betegek több napot töltöttek intézetben, mint a nem hypertoniás egyének (10,9±6,8 vs. 9,2±6,3). A felvételkor mért spontán O2-szaturáció a hypertoniás beteg között alacsonyabb volt, mint a nem hypertoniás egyéneknél (92,6±4,8 vs. 89,6±9,7%), ARB-t szedők között volt a legalacsonyabb (87,5±8,5%, p=0,01 vs. nem hypertoniás). Az összes halálozás a hypertoniás betegek között 8,7%, a nem hypertoniás betegek között csak 1,6% volt. A hypertoniás betegeket vizsgálva, a RAS-gátlót szedők és nem szedők között az intenzív ellátás 16,3 vs. 13%. Az összes halálozás a RAS-gátlót szedők között 10%, a nem szedők között 4,3%, az ARB-t szedők között 13% és az ACE-gátlót szedők között 8,8% volt. Eredményeink alapján a hypertonia nagyobb kockázatot jelenthet a rosszabb Covid-kimenetel tekintetében. A RAS-gátló kezelésben részesültek nagyobb kockázatát észleltük a RAS-gátló kezelésben nem részesültekhez képest.

    A Covid-19-járvánnyal járó problémák a krónikusbeteg-populációkra vannak a legrosszabb hatással. A stressz, a mindennapi rutin felbomlása, családi problémák, a megbetegedés és az ellátás nehezítettsége miatt a krónikus fájdalommal élők az egyik kiemelkedően veszélyeztetett csoport. Ezért elengedhetetlen felmérni ennek a csoportnak a helyzetét, pszichés jóllétét. A jelen kérdőíves felmérés célja a következő volt: a krónikus fájdalommal élő személyek felmérése a Covid-19-pan­dé­mia alatt, kitérve a fájdalommal összefüggésbe hozható pszichés háttértényezőkre, mint a depresszió, az érzelemszabályozás, az alexithymia, a jóllét, az egészségértés és a társas támogatás. A felmérés 158 főt vizsgált, akiknél legalább 3 hónapja állt fenn valamilyen fájdalom, de orvosi ellátásban nem részesültek. Az adatfelvétel két időpontban zaj­lott: 2021. februárban és decemberben. A pandémiára való tekintettel a felmérés online, kérdőíves módszerrel zajlott. A felmérés során a következő hat pszichológiai tesztet alkalmaztuk: Torontói Alexithymia Skála, Beck Dep­resszió Kérdőív rövidített változata, Érzelemszabályozási Nehézségek Kérdőív, Multidimenzionális Észlelt Társas Támogatás Kérdőív, az egészségértést mérő Chew-kér­dé­sek és a WHO Jóllét Kérdőív. A vizsgálati személyek életkora 20–80 év közötti, 140 (88%) nő. Negyvenkét fő (27%) érte el a súlyos alexithymia mértékét. Depresszió 118 főnél (75%) volt kimutatható, melyből 72 fő (46%) enyhe, 26 fő (16%) közepesen súlyos és 20 fő (13%) súlyos depresszióval érintett. A fájdalom mértéke és az alexithymia [r(158) = 0,16, p = 0,004], a depresszió [r(158) = 0,41, p < 0,001], az érzelemszabályozási nehézségek [r(158) = 0,26, p = 0,004], az egészségértési és érzelemszabályozási nehézségek [r(158) = 0,25, p = 0,001] között pozitív, szignifikáns korreláció volt kimutatható. A fájdalommal élő személyeket jellemző komorbiditások (szorongás, hangulatzavar, alvászavar) mellett a járvány indukálta hosszan tartó szociális izoláció, stressz és a félelem magyarázhatja a depresszió, az ér­ze­lemszabályozási nehézségek és az egészségértési problémák magas arányát a vizsgált mintában, amik tovább súlyosbítják az alexithymia és a fájdalom mértékét. Ezek alapján fontos felhívni a szakemberek figyelmét az érintett személyek megfelelő egészségügyi ellátására és edukációs szükségleteire.

    The pandemic has affected social relations in many ways, and even created new social groups through vaccination. The goal of this study was to explore whether intergroup discrimination regarding vaccination status can be observed in a resource allocation task. Participants (N = 818; Mage = 46.0 years) completed a resource allocation task. Results showed that the better-than-average effect was widespread among the participants as most of them perceived themselves more informed about the pandemic than others. The resource allocation task showed participants preferred to create maximum difference in favor of their group in intergroup situations, but decisions were fairer when targets' group membership was identical. Moreover, vaccinated people were more likely to use maximum difference strategies than non-vaccinated people. The results revealed that vaccination status changes the perception of intergroup situations, which may be important in planning future strategies to handle mass emergencies similar to the current pandemic.
