Günümüzde COVID-19 ile ilgili çalışmalar giderek artmakta ve her geçen gün bilgiler değişmektedir. Bizim de bu çalışmadaki amacımız COVID-19 tanısı ile hastanede yatan hastaların yatış süresine etki eden faktörlerin incelenmesidir. Çalışmaya Mart 2020 ve Haziran 2020 tarihleri arasında hastanemiz enfeksiyon hastalıkları servisinde yatan 102 hasta dahil edildi. Hastaların yaşı ve cinsiyeti, komorbid hastalıkları, vital bulguları, yatış esnasındaki lökosit, nötrofil, lenfosit, monosit, hemoglobin (Hb), trombosit (PLT), ortalama trombosit hacmi (MPV), aspartat aminotransferaz (AST), alanin aminotransferaz (ALT) üre, kreatin, C-reaktif protein (CRP), fibrinojen, laktat dehidrojenaz (LDH), kreatinin, protrombin zamanı (PT), uluslararası normalleştirilmiş oran (INR), procalsitonin (pct), D-dimer, ferritin, troponin, nötrofil/lenfosit oranı (NLR) trombosit-lenfosit oranı (PLR), monosit-lenfosit oranı (MLR) ve MPV-lenfosit oranı (MPVLR) değerleri retrospektif olarak incelendi. COVID-19 PCR ve akciğer tomografilerindeki parankimal tutulumları sınıflandırıldı. Aldıkları antiviral, antibiyotik ve diğer destek tedavileri ve hastanede yatış süreleri kayıt edildi. İstatistiksel analiz için verilerin dağılımı Kolmogorov-Simirnov testi ile incelendi. İstatistiksel analizler SPSS v.22 paket programı ile yapıldı, anlamlılık düzeyi 0,05 olarak dikkate alındı. Çalışmamızda 51 erkek ve 51 kadın toplam 102 hasta değerlendirildi. Hastaların yaş ortalaması 56,51±15,48 yıl olarak bulundu. Ortalama hastane yatış süresi ortalama 7,58±3,35 gündü. DM, HT ve malignitesi olan hastalarda ve favipravir, enoksaparin ve vitamin C desteği alan hastalarda daha uzun hastane yatışı olduğu görüldü. Uzun süre hastane yatışı olan hastalarda ateş, procalsitonin, AST, LDH değerlerinin kısa yatış süresi olan hastalara göre anlamlı olarak daha yüksek olduğu saptandı. Lenfosit sayısı ve yüzdesi, NLR, monosit sayısı, MPV/lenfosit oranı değerleri ise yedi günden uzun süre yatan hastalarda anlamlı düşük bulundu. iii The new coronavirus disease (COVID-19), which emerged towards the end of 2019 and caused a pandemic all over the world, infected millions of people all over the world in 2020. Nowadays, studies on COVID-19 are increasing and information changes day by day. Our aim in this study is to determine the factors affecting the length of stay of patients hospitalized with the diagnosis of COVID-19. 102 patients hospitalized in our hospital's infection ward between March 2020 and June 2020 were included in the study. Age, gender, comorbid diseases, vital signs, leukocyte, neutrophil, lymphocyte, monocyte, hemoglobin, platelet, mean platelet volume (MPV), aspartate aminotransferase (AST), alanine aminotransferase (ALT) urea, creatine, c-reactive protein (CRP), procalcitonin, d-dimer, fibrinogen, lactate dehydrogenase (LDH), creatinine, prothrombin time (PT), international normalized ratio (INR), procalcitonin (pct), d-dimer, ferritin, troponin, neutrophil / lymphocyte ratio (NLR) platelet-lymphocyte ratio (PLR), monocyte-lymphocyte ratio (MLR) and MPV-lymphocyte ratio values were analyzed retrospectively. Parenchymal involvement in COVID-19 PCR, hospitalization tomography were classified. The antiviral, antibiotic and other supportive treatments they received and the length of their hospital stay were included. For statistical analysis, the distribution of the data was examined using the Kolmogorov-Simirnov test. Statistical analyzes were made with SPSS v.22 package program, the level of significance was taken into account as 0.05. 51 male and 51 female patients were evaluated in our study. The mean age of the patients was 56.51 ± 15.48 years. Average hospital stay was 7.58 ± 3.35 days. The patients were evaluated as longer and shorter hospitalizations than five days, and longer and shorter hospitalizations than seven days. Longer hospital stays were observed in patients with DM, HT, malignancy and those receiving favipravir, enoxaparin and vitamin C supplements. Fever, procalcitonin, AST and LDH values iv were found to be significantly higher in patients who were hospitalized for a long time compared to patients with a short hospitalization period. Lymphocyte count and percentage, neutrophil-lymphocyte ratio, monocyte count, mean platelet volume (MPV) lymphocyte ratio values were found to be significantly lower in long hospitalized patients.

Corticosteroid therapy has been used for many years and is still controversial in use. In the Coronovirus disease2019 (COVID-19) outbreak, it is very important to find reliable treatment information for clinicians andpatients. Low dose corticosteroids are also used in patients with septic shock, relative adrenal insufficiency andacute respiratory distress syndrome (ARDS). There is not enough evidence for its use in COVID-19. However,low dose use is recommended in the case of ARDS and septic shock in COVID-19 patients. Caution should beexercised until further evidence emerges surrounding the use of corticosteroids in COVID-19 patients.

Among the several explanations proposed, an early and large testing of the population was put forward. Most current debates on COVID-19 focus on the differences among countries, but little attention has been given to regional differences and diet. The low-death rate European countries (e.g. Austria, Baltic States, Czech Republic, Finland, Norway, Poland, Slovakia) have used different quarantine and/or confinement times and methods and none have performed as many early tests as Germany. Among other factors that may be significant are the dietary habits. It seems that some foods largely used in these countries may reduce angiotensin-converting enzyme activity or are anti-oxidants. Among the many possible areas of research, it might be important to understand diet and angiotensin-converting enzyme-2 (ACE2) levels in populations with different COVID-19 death rates since dietary interventions may be of great benefit.

Due to COVID-19, a global pandemic was declared by WHO on March 11, 2020, and as a result, curfews were applied in our country. Objective: The aim of this study is to evaluate the effects of restrictive measures applied during the COVID-19 pandemic on children's adherence to the gluten-free diet. Method: The research was carried out in Ege University Faculty of Medicine, Department of Pediatric Gastroenterology, Hepatology and Nutrition. Fifty patients between the ages of 2 and 18 who were diagnosed with celiac disease and followed a gluten-free diet for at least 2 years were included in the study. Demographic data of the cases, body weight, height, body mass index values and standard deviation scores, tTG-IgA levels before and during the pandemic were recorded from the outpatient follow-up files and the hospital data system. Patients with serologically tTG-IgA levels above 20 U/ml were considered to have dietary compliance problems. A questionnaire was prepared verbally questioning the patients' compliance with the gluten-free diet and the factors that may affect it during the pandemic period. This questionnaire was filled in face to face during the outpatient follow-ups and with telephone interviews. Results: In our study, 31 (62%) of 50 celiac patients were female and 19 (38%) were male. The mean age at diagnosis is 11,93 ± 4,06 years. The three most common complaints at the time of diagnosis were growth retardation (56%), abdominal pain (46%), and diarrhea (36%). When our patients were evaluated anthropometrically before and after the pandemic; A statistically significant increase was found in body weight SDSs (p=0.006). A significant increase was found in height SDSs (p=0.01). There was an increase in BMI SDSs, but it was not statistically significant (p>0.05). While 64% of patients had negative tTG-IgA antibodies before the pandemic, this rate decreased to 56% during the pandemic, but no statistically significant difference was found (p=0.07). When dietary compliance was questioned verbally, 49 patients in our sample reported that they adhered to the diet before and after the pandemic. When the degree of adherence to the diet was questioned, 37 patients stated that they always adhered to the diet before and after the pandemic. When the tTG-IgA levels of these patients were compared before and after the pandemic, it was observed that there was an increase in antibody levels, but no statistically significant difference was found. This showed that the patient's statement was unreliable. When the frequency of eating out was questioned, a statistically significant decrease was observed during the pandemic compared to the pre-pandemic period (p=0.001). There was a decrease in the monthly income of the families during the pandemic, which was statistically significant (p=0.04). Before and during the pandemic, 45 patients stated that they had difficulty in supplying gluten-free food. While the most common reasons for this difficulty before the pandemic were that gluten-free products were expensive and not available in every market, the concern of being infected with COVID-19 and curfew were added to these during the pandemic. Conclusion: During the pandemic, there was an increase in the body weight and height SDS of the patients. This increase does not support the serological response. This suggests that the occurrence of inflammation and intestinal damage requires longer follow-up, and the time between pre-pandemic and pre-pandemic control examinations may be insufficient. Giriş: Çölyak hastalığı, genetik yatkınlığı olan bireylerde glüten içeren yiyeceklerin tüketilmesi sonucu ortaya çıkan bir enteropatidir ve tedavisi ömür boyu sürecek glütensiz diyettir. COVID-19 nedeniyle 11 Mart 2020’de DSÖ tarafından küresel pandemi ilan edilmiştir ve bunun sonucunda ülkemizde sokağa çıkma kısıtlamaları uygulanmıştır. Amaç: Bu çalışmanın amacı COVID-19 pandemisi sırasında uygulanan kısıtlayıcı önlemlerin çocukların glütensiz diyete uyumları üzerine etkilerinin değerlendirilmesidir. Yöntem: Araştırma Ege Üniversitesi Tıp Fakültesi Pediatrik Gastroenteroloji, Hepatoloji ve Beslenme Bilim Dalında yapıldı. Çölyak hastalığı tanısı almış ve en az 2 yıl süreyle glütensiz diyet uygulayan 2-18 yaş aralığındaki 50 hasta çalışmaya dahil edildi. Olguların demografik verileri, pandemi öncesindeki ve pandemi sırasındaki vücut ağırlığı, boy, vücut kitle indeksi değerleri ve standart deviasyon skorları, tTG-IgA düzeyleri poliklinik izlem dosyalarından ve hastane veri sisteminden kaydedildi. Serolojik olarak tTG-IgA düzeyi 20 U/ml’nin üstünde olan hastaların diyete uyum sorunu olduğu kabul edildi. Hastaların glütensiz diyete uyumunu ve pandemi döneminde buna etki edebilecek faktörleri sözel olarak sorgulayan bir anket düzenlendi. Bu anket formu hastaların poliklinik izlemlerinde yüz yüze ve telefon görüşmesiyle dolduruldu. Bulgular: Çalışmamızda 50 çölyaklı olgunun 31’si (%62) kız, 19’i (%38) erkek idi. Ortalama yaşı 11,93 ± 4,06 yıldır. Tanı anındaki en sık üç yakınma büyüme geriliği (%56), karın ağrısı (%46), ishal (%36) idi. Hastalarımız antropometrik olarak pandemi öncesi ve sonrası değerlendirildiğinde; vücut ağırlığı SDS’lerinde istatistiksel olarak anlamlı artış saptandı (p=0,006). Boy SDS’lerinde anlamlı artış bulundu (p=0,01). VKİ SDS’lerinde artış olduğu görüldü fakat istatistiksel olarak anlamlı değildi (p>0,05). Pandemi öncesi %64 hastanın tTG-IgA antikoru negatifken bu oran pandemi sırasında %56’ya düşmüştü, ancak istatistiksel olarak anlamlı fark saptanmadı (p=0,07). Diyete uyum sözel olarak sorgulandığında örneklemimizdeki 49 hasta pandemiden önce ve sonra diyete uyduğunu bildirdi. Diyete uyum derecesi sorgulandığında 37 hasta pandemiden önce ve sonra diyete daima uyduğunu belirtti. Bu hastaların pandemi öncesi ve sonrası tTG-IgA düzeyleri kıyaslandığında antikor düzeylerinde artış olduğu görüldü fakat istatistiksel olarak anlamlı fark saptanmadı. Bu da hastaların beyanının güvenilir olmadığını gösterdi. Dışarıda yemek yeme sıklığı sorgulandığında pandemi sırasında, pandemi öncesine göre istatistiksel olarak anlamlı düzeyde azalma görüldü (p=0,001). Pandemi süresince ailelerin aylık gelirinde düşüş görüldü, istatistiksel olarak anlamlı bulundu (p=0,04). Pandemi öncesi ve pandemi sırasında 45 hasta glütensiz gıdayı tedarik etmekte zorlandığını belirtti. Pandemiden önce bu zorlanmanın en sık nedenleri glütensiz ürünlerin pahalı olması ve her markette bulunmaması iken, pandemi süresince bunlara COVID-19 ile enfekte olma endişesi ve sokağa çıkma yasağı da eklendi. Sonuç: Pandemi süresince hastaların vücut ağırlığı ve boy SDS’lerinde artış olmuştur. Bu artışı serolojik yanıt desteklememektedir. Bu durum inflamasyon ve bağırsaktaki hasarın ortaya çıkmasının daha uzun takip gerektirdiğini hastaların pandemi öncesi ve pandemi sırasındaki kontrol muayeneleri arasındaki sürenin yetersiz olabileceğini düşündürmektedir.

Günümüzde COVID-19 ile ilgili çalışmalar giderek artmakta ve her geçen gün bilgiler değişmektedir. Bizim de bu çalışmadaki amacımız COVID-19 tanısı ile hastanede yatan hastaların yatış süresine etki eden faktörlerin incelenmesidir. Çalışmaya Mart 2020 ve Haziran 2020 tarihleri arasında hastanemiz enfeksiyon hastalıkları servisinde yatan 102 hasta dahil edildi. Hastaların yaşı ve cinsiyeti, komorbid hastalıkları, vital bulguları, yatış esnasındaki lökosit, nötrofil, lenfosit, monosit, hemoglobin (Hb), trombosit (PLT), ortalama trombosit hacmi (MPV), aspartat aminotransferaz (AST), alanin aminotransferaz (ALT) üre, kreatin, C-reaktif protein (CRP), fibrinojen, laktat dehidrojenaz (LDH), kreatinin, protrombin zamanı (PT), uluslararası normalleştirilmiş oran (INR), procalsitonin (pct), D-dimer, ferritin, troponin, nötrofil/lenfosit oranı (NLR) trombosit-lenfosit oranı (PLR), monosit-lenfosit oranı (MLR) ve MPV-lenfosit oranı (MPVLR) değerleri retrospektif olarak incelendi. COVID-19 PCR ve akciğer tomografilerindeki parankimal tutulumları sınıflandırıldı. Aldıkları antiviral, antibiyotik ve diğer destek tedavileri ve hastanede yatış süreleri kayıt edildi. İstatistiksel analiz için verilerin dağılımı Kolmogorov-Simirnov testi ile incelendi. İstatistiksel analizler SPSS v.22 paket programı ile yapıldı, anlamlılık düzeyi 0,05 olarak dikkate alındı. Çalışmamızda 51 erkek ve 51 kadın toplam 102 hasta değerlendirildi. Hastaların yaş ortalaması 56,51±15,48 yıl olarak bulundu. Ortalama hastane yatış süresi ortalama 7,58±3,35 gündü. DM, HT ve malignitesi olan hastalarda ve favipravir, enoksaparin ve vitamin C desteği alan hastalarda daha uzun hastane yatışı olduğu görüldü. Uzun süre hastane yatışı olan hastalarda ateş, procalsitonin, AST, LDH değerlerinin kısa yatış süresi olan hastalara göre anlamlı olarak daha yüksek olduğu saptandı. Lenfosit sayısı ve yüzdesi, NLR, monosit sayısı, MPV/lenfosit oranı değerleri ise yedi günden uzun süre yatan hastalarda anlamlı düşük bulundu. iii The new coronavirus disease (COVID-19), which emerged towards the end of 2019 and caused a pandemic all over the world, infected millions of people all over the world in 2020. Nowadays, studies on COVID-19 are increasing and information changes day by day. Our aim in this study is to determine the factors affecting the length of stay of patients hospitalized with the diagnosis of COVID-19. 102 patients hospitalized in our hospital's infection ward between March 2020 and June 2020 were included in the study. Age, gender, comorbid diseases, vital signs, leukocyte, neutrophil, lymphocyte, monocyte, hemoglobin, platelet, mean platelet volume (MPV), aspartate aminotransferase (AST), alanine aminotransferase (ALT) urea, creatine, c-reactive protein (CRP), procalcitonin, d-dimer, fibrinogen, lactate dehydrogenase (LDH), creatinine, prothrombin time (PT), international normalized ratio (INR), procalcitonin (pct), d-dimer, ferritin, troponin, neutrophil / lymphocyte ratio (NLR) platelet-lymphocyte ratio (PLR), monocyte-lymphocyte ratio (MLR) and MPV-lymphocyte ratio values were analyzed retrospectively. Parenchymal involvement in COVID-19 PCR, hospitalization tomography were classified. The antiviral, antibiotic and other supportive treatments they received and the length of their hospital stay were included. For statistical analysis, the distribution of the data was examined using the Kolmogorov-Simirnov test. Statistical analyzes were made with SPSS v.22 package program, the level of significance was taken into account as 0.05. 51 male and 51 female patients were evaluated in our study. The mean age of the patients was 56.51 ± 15.48 years. Average hospital stay was 7.58 ± 3.35 days. The patients were evaluated as longer and shorter hospitalizations than five days, and longer and shorter hospitalizations than seven days. Longer hospital stays were observed in patients with DM, HT, malignancy and those receiving favipravir, enoxaparin and vitamin C supplements. Fever, procalcitonin, AST and LDH values iv were found to be significantly higher in patients who were hospitalized for a long time compared to patients with a short hospitalization period. Lymphocyte count and percentage, neutrophil-lymphocyte ratio, monocyte count, mean platelet volume (MPV) lymphocyte ratio values were found to be significantly lower in long hospitalized patients.

Corticosteroid therapy has been used for many years and is still controversial in use. In the Coronovirus disease2019 (COVID-19) outbreak, it is very important to find reliable treatment information for clinicians andpatients. Low dose corticosteroids are also used in patients with septic shock, relative adrenal insufficiency andacute respiratory distress syndrome (ARDS). There is not enough evidence for its use in COVID-19. However,low dose use is recommended in the case of ARDS and septic shock in COVID-19 patients. Caution should beexercised until further evidence emerges surrounding the use of corticosteroids in COVID-19 patients.

Among the several explanations proposed, an early and large testing of the population was put forward. Most current debates on COVID-19 focus on the differences among countries, but little attention has been given to regional differences and diet. The low-death rate European countries (e.g. Austria, Baltic States, Czech Republic, Finland, Norway, Poland, Slovakia) have used different quarantine and/or confinement times and methods and none have performed as many early tests as Germany. Among other factors that may be significant are the dietary habits. It seems that some foods largely used in these countries may reduce angiotensin-converting enzyme activity or are anti-oxidants. Among the many possible areas of research, it might be important to understand diet and angiotensin-converting enzyme-2 (ACE2) levels in populations with different COVID-19 death rates since dietary interventions may be of great benefit.

Due to COVID-19, a global pandemic was declared by WHO on March 11, 2020, and as a result, curfews were applied in our country. Objective: The aim of this study is to evaluate the effects of restrictive measures applied during the COVID-19 pandemic on children's adherence to the gluten-free diet. Method: The research was carried out in Ege University Faculty of Medicine, Department of Pediatric Gastroenterology, Hepatology and Nutrition. Fifty patients between the ages of 2 and 18 who were diagnosed with celiac disease and followed a gluten-free diet for at least 2 years were included in the study. Demographic data of the cases, body weight, height, body mass index values and standard deviation scores, tTG-IgA levels before and during the pandemic were recorded from the outpatient follow-up files and the hospital data system. Patients with serologically tTG-IgA levels above 20 U/ml were considered to have dietary compliance problems. A questionnaire was prepared verbally questioning the patients' compliance with the gluten-free diet and the factors that may affect it during the pandemic period. This questionnaire was filled in face to face during the outpatient follow-ups and with telephone interviews. Results: In our study, 31 (62%) of 50 celiac patients were female and 19 (38%) were male. The mean age at diagnosis is 11,93 ± 4,06 years. The three most common complaints at the time of diagnosis were growth retardation (56%), abdominal pain (46%), and diarrhea (36%). When our patients were evaluated anthropometrically before and after the pandemic; A statistically significant increase was found in body weight SDSs (p=0.006). A significant increase was found in height SDSs (p=0.01). There was an increase in BMI SDSs, but it was not statistically significant (p>0.05). While 64% of patients had negative tTG-IgA antibodies before the pandemic, this rate decreased to 56% during the pandemic, but no statistically significant difference was found (p=0.07). When dietary compliance was questioned verbally, 49 patients in our sample reported that they adhered to the diet before and after the pandemic. When the degree of adherence to the diet was questioned, 37 patients stated that they always adhered to the diet before and after the pandemic. When the tTG-IgA levels of these patients were compared before and after the pandemic, it was observed that there was an increase in antibody levels, but no statistically significant difference was found. This showed that the patient's statement was unreliable. When the frequency of eating out was questioned, a statistically significant decrease was observed during the pandemic compared to the pre-pandemic period (p=0.001). There was a decrease in the monthly income of the families during the pandemic, which was statistically significant (p=0.04). Before and during the pandemic, 45 patients stated that they had difficulty in supplying gluten-free food. While the most common reasons for this difficulty before the pandemic were that gluten-free products were expensive and not available in every market, the concern of being infected with COVID-19 and curfew were added to these during the pandemic. Conclusion: During the pandemic, there was an increase in the body weight and height SDS of the patients. This increase does not support the serological response. This suggests that the occurrence of inflammation and intestinal damage requires longer follow-up, and the time between pre-pandemic and pre-pandemic control examinations may be insufficient.
