    Abstract: Background: Grading schemes for severity of suspected allergic reactions have been applied to the perioperative setting, but there is no scoring system that estimates the likelihood that the reaction is an immediate hypersensitivity reaction. Such a score would be useful in evaluating current and proposed tests for the diagnosis of suspected perioperative immediate hypersensitivity reactions and culprit agents. Methods: We conducted a Delphi consensus process involving a panel of 25 international multidisciplinary experts in suspected perioperative allergy. Items were ranked according to appropriateness (on a scale of 1-9) and consensus, which informed development of a clinical scoring system. The scoring system was assessed by comparing scores generated for a series of clinical scenarios against ratings of panel members. Supplementary scores for mast cell tryptase were generated. Results: Two rounds of the Delphi process achieved stopping criteria for all statements. From an initial 60 statements, 43 were rated appropriate (median score 7 or more) and met agreement criteria (disagreement index <0.5); these were used in the clinical scoring system. The rating of clinical scenarios supported the validity of the scoring system. Although there was variability in the interpretation of changes in mast cell tryptase by the panel, we were able to include supplementary scores for mast cell tryptase. Conclusion: We used a robust consensus development process to devise a clinical scoring system for suspected perioperative immediate hypersensitivity reactions. This will enable objectivity and uniformity in the assessment of the sensitivity of diagnostic tests.

    Tobago is a small island on the southeast edge of the Caribbean Plate with a continental flora and fauna. Using DNA sequences from Genbank, new sequences, and morphological data from the snakesErythrolamprusepinephalus,E.melanotus,E.reginae, andE.zweifeli, the species status of specimens of a Tobago snake previously considered to beErythrolamprusreginaewas assessed.Erythrolampruszweifeli, long considered a subspecies ofE.reginae, was found to be a northern Venezuela-Trinidad endemic and the sister toE.reginae. The trans-Andean speciesE.epinephalusis shown to be non-monophyletic while the Costa Rican lineage ofE.epinephalusis weakly supported as the sister to the Tobago population. The TobagoErythrolamprusis described as a distinct taxon based upon five specimens from four localities in lower montane rainforest. Much of the new species range includes the Main Ridge Forest Reserve of Tobago, the oldest protected forest in the Western Hemisphere. All known locations fall within a 400-ha area, and its total geographic distribution is likely to be less than 4,566 ha. The restricted distribution of this new snake makes it a likely candidate for threatened status. The new species also becomes another biogeographic link between northern Venezuela and Tobago.

    The success of the ambitious programs of both long- and short-baseline neutrino-oscillation experiments employing liquid-argon time-projection chambers will greatly rely on the precision with which the weak response of the argon nucleus can be estimated. In the E12-14-012 experiment at Jefferson Lab Hall A, we have studied the properties of the argon nucleus by scattering a high-quality electron beam off a high-pressure gaseous argon target. Here, we present the measured $^{40}$Ar$(e,e^{\prime})$ double differential cross section at incident electron energy $E=2.222$~GeV and scattering angle $\theta = 15.541^\circ$. The data cover a broad range of energy transfers, where quasielastic scattering and delta production are the dominant reaction mechanisms. The result for argon is compared to our previously reported cross sections for titanium and carbon, obtained in the same kinematical setup. Comment: 6 pages, 4 figures

    Introduction: Maternal abdominal decompression during pregnancy could be used in an attempt to improve utero-placental blood flow. We utilized Doppler ultrasonography to investigate the effects of this procedure on blood flow in the umbilical artery and fetal middle cerebral artery. Methods: Women (n = 23) with singleton pregnancies attending antenatal abdominal decompression were enrolled in the study. Doppler velocity waveforms were obtained from umbilical artery and fetal middle cerebral artery before and after a 30-minute decompression session. Resistance indices were compared using the Student's t - test (p < 0.05 significant). Results: 23 healthy pregnant women were included at an average gestational age of 36+1 weeks. The mean resistance index before decompression in the umbilical artery was 0.58 (s = 0.10) and after decompression 0.54 (s = 0.07, p = 0.06). In the middle cerebral artery the values were 0.72 (s = 0.11) and 0.77 (s = 0.08), respectively (p = 0.01). Discussion and conclusion: Significantly increased resistance index in the middle cerebral artery implies a higher fetal brain oxygenation after decompression. This is further corroborated by the trend towards a decrease in umbilical artery resistance index. The effects of decompression could be beneficial in pregnancies complicated by IUGR or hypertensive disorders of pregnancy.

    New tools and methods for investigating the brain have given neuroscientists a chance to examine the working brain and placed neuroscience as the central discipline in cognitive science. My main goal in this article is to examine theoretical and philosophical assumptions on which different understandings and interpretations of neuroscientific research are based and to show why philosophical reflection on neuroscience is needed. I first discuss different roles philosophy potentially plays in cognitive science. After a short presentation of Descartes' position concerning the mind body problem and cognitive science approaches to answer his challenge, I examine different theoretical frameworks for neuroscientific research.

    Abstract Background and Purpose: The term ‘social media’ refers to a cluster of applications and online services that support human interaction and content broadcasting and sharing. Current services are isolated islands or ‘walled gardens’, and are based on a business model that is highly exploitative of individuals and their data. Design/Methodology/Approach: Surveys of the refereed literature have been undertaken on several occasions during the period 2012-13. Reviews were conducted of social media services that are reasonably described as ‘consumer-oriented’. Media reports on those services were uncovered. The available information was then subjected to analysis, including reflection based on prior research conducted by the author. Results: Required characteristics of consumer-oriented social media, and barriers to emergence and adoption of such services were identified. That provided a basis for proposing means to overcome those barriers. Key impediments to the emergence of such services were identified, and means of overcoming the impediments outlined. Conclusion: An alternative, consumer-oriented approach is feasible, involving open architecture, inter-operability and portability features, fair terms and privacy-sensitivity.
