    Description: Rheumatoid arthritis (RA) is a chronic inflammatory disease that causes joint inflammation, bone and cartilage destruction, and sometimes disability. The pathophysiology of RA, on the other hand, is multifactorial and, to a significant extent, unknown. Abnormal glycine metabolism is well established pathophysiological feature in RA [1] and the circulatory levels of glycine are largely reported to be elevated in RA [2]. Further, various clinical studies report the decreased circulatory levels of histidine in RA Patients. The low free serum histidine levels have also been found to be associated with disease activity in RA [3]. Other studies reported higher levels of free histidine in the synovial fluid (SF) compared to that in the patient serum samples of RA (considered to be an inflammatory arthritis condition) but SF histidine levels were significantly lower than corresponding results in patients with osteoarthritis (OA, considered to a non-inflammatory arthritis condition) [4]. Therefore, we hypothesized and demonstrated that the circulatory glycine to histidine ratio (GHR) is significantly elevated in RA compared to NC (with p-value <0.0001). For this, fifty (N= 50) serum samples obtained from thirty (N=30) normal control rats and twenty (N=20) rat models of rheumatoid arthritis (RA, induced using Complete Freund's adjuvant (CFA) treatment) were analyzed using high-resolution 800 MHz NMR spectrometer. The serum metabolic profiles were measured using standard 1D-1H-CPMG NMR experiments and the concentration levels of selected metabolites (glycine and histidine) were estimated using NMR suite of commercial software CHENOMX following the procedure as described previously from our research group [5]. The mean values (±standard deviation) of circulatory GHR levels in the serum samples of RA and NC rats were found to be 6.65 ± 2.29 and 3.46 ± 0.93, respectively. We further performed the receiver operating characteristic (ROC) curve analysis and the resulted area under ROC curve value of 0.97 (with 95% confidence interval 0.92-1.0 and p-value <0.0001) suggested that the circulatory GHR levels have significant potential to serve as a better preclinical indicator of disease activity in RA and may be a reliable clinical biomarker for monitoring the treatment response in such preclinical models.     Background: In pediatric age the prevalence of obesity is high. Obese children who do not have other risk factors than excess weight have been defined as "metabolically healthy obese" (MHO). Aim: The aim of this study is to evaluate, in a population of obese children, the prevalence of the MHO and "metabolically unhealthy obese" (MUO) phenotype. Furthermore, we evaluated the distribution of Uric Acid, HOMA index and Waist-Height ratio (W-Hr) in the MHO and MUO sub-groups and the impact of these non-traditional risk factors on the probability to be MUO. Methods:In 1201 obese children and adolescents [54% males, age (±SD) 11.9 (±3.0) years] weight, height, waist circumference, systolic (SBP) and diastolic (DBP) blood pressure, pubertal status, glucose, insulin, HDL cholesterol, triglycerides and Uric Acid serum values were assessed. MUO phenotype was defined as the presence of at least one of the following risk factors: SBP or DBP ≥ 90th percentile, glycaemia ≥ 100 mg/dl, HDL cholesterol <40 mg/dl, triglycerides ≥100 mg/dl (children <10 years) or ≥130 mg/dl (children ≥10 years). A multivariate logistic regression analysis was used to estimate the association between MUO phenotype and non-traditional cardiovascular risk factors. Results: The prevalence of the MUO status was high (61%). MUO subjects were more often male, older and pubertal (p < 0.001). The levels of the three non-traditional risk factors were significantly higher in MUO children compared to MHO children (p < 0.001) and all of them were independent predictors of the fact of being MUO [OR 1.41 (95% CI 1.24-1.69); 1.15 (95% CI 1.06-1.23) and 1.03 (95% CI1.01-1.05) for Uric Acid, HOMA index and W-Hr, respectively]. About 15% of MHO subjects had serum Uric Acid, HOMA index and W-Hr values within the highest quartile of the study population. Conclusion: The prevalence of MUO subjects in a large pediatric population is high and serum Uric Acid, HOMA index and W-Hr values are independent predictors of the probability of being MUO. A non-negligible percentage of subjects MHO has high values of all three non-traditional risk factors.

Inertial measurement units (IMUs) enable easy to operate and low-cost data recording for gait analysis. When combined with treadmill walking, a large number of steps can be collected in a controlled environment without the need of a dedicated gait analysis laboratory. In order to evaluate existing and novel IMU-based gait analysis algorithms for treadmill walking, a reference dataset that includes IMU data as well as reliable ground truth measurements for multiple participants and walking speeds is needed. This dataset contains data from of 15 healthy young adults who walked on a treadmill at three different speeds. Data was acquired using seven IMUs placed on the lower body, two different reference systems (Zebris FDMT-HQ and OptoGait) and two RGB cameras. Additionally, in order to validate an existing IMU-based gait analysis algorithm by using the dataset, an adaptable modular data analysis pipeline was built. Our results show close agreement between the Zebris and OptoGait systems (r=0.99), demonstrating the high quality of our reference data.

This is the abstract: Background: The prevalence and effects of cigarette smoking and cannabis use in persons with congenital heart disease (CHD) are poorly understood. We (1) described the prevalence of cigarette smoking, cannabis consumption, and co-use in adults with CHD; (2) investigated intercountry differences; (3) tested the relative effects on physical functioning, mental health, and quality of life (QOL); and (4) quantified the differential effect of cigarette smoking, cannabis use, or co-use on those outcomes. Methods: APPROACH-IS was a cross-sectional study, including 4028 adults with CHD from 15 countries. Patients completed questionnaires to measure physical functioning, mental health, and QOL. Smoking status and cannabis use were assessed by means of the Health Behaviour Scale-Congenital Heart Disease. Linear models with doubly robust estimations were computed after groups were balanced with the use of propensity weighting. Results: Overall, 14% of men and 11% of women smoked cigarettes only; 8% of men and 4% of women consumed cannabis only; and 4% of men and 1% of women used both substances. Large intercountry variations were observed, with Switzerland having the highest prevalence for smoking cigarettes (24% of men, 19% of women) and Canada the highest for cannabis use (19% of men, 4% of women). Cigarette smoking had a small negative effect on patient-reported outcomes, and the effect of cannabis was negligible. The effect of co-use was more prominent, with a moderate negative effect on mental health. Conclusions: We found significant intercountry variability in cigarette and cannabis use in adults with CHD. Co-use has the most detrimental effects on patient-reported outcomes.

    Description: Rheumatoid arthritis (RA) is a chronic inflammatory disease that causes joint inflammation, bone and cartilage destruction, and sometimes disability. The pathophysiology of RA, on the other hand, is multifactorial and, to a significant extent, unknown. Abnormal glycine metabolism is well established pathophysiological feature in RA [1] and the circulatory levels of glycine are largely reported to be elevated in RA [2]. Further, various clinical studies report the decreased circulatory levels of histidine in RA Patients. The low free serum histidine levels have also been found to be associated with disease activity in RA [3]. Other studies reported higher levels of free histidine in the synovial fluid (SF) compared to that in the patient serum samples of RA (considered to be an inflammatory arthritis condition) but SF histidine levels were significantly lower than corresponding results in patients with osteoarthritis (OA, considered to a non-inflammatory arthritis condition) [4]. Therefore, we hypothesized and demonstrated that the circulatory glycine to histidine ratio (GHR) is significantly elevated in RA compared to NC (with p-value <0.0001). For this, fifty (N= 50) serum samples obtained from thirty (N=30) normal control rats and twenty (N=20) rat models of rheumatoid arthritis (RA, induced using Complete Freund's adjuvant (CFA) treatment) were analyzed using high-resolution 800 MHz NMR spectrometer. The serum metabolic profiles were measured using standard 1D-1H-CPMG NMR experiments and the concentration levels of selected metabolites (glycine and histidine) were estimated using NMR suite of commercial software CHENOMX following the procedure as described previously from our research group [5]. The mean values (±standard deviation) of circulatory GHR levels in the serum samples of RA and NC rats were found to be 6.65 ± 2.29 and 3.46 ± 0.93, respectively. We further performed the receiver operating characteristic (ROC) curve analysis and the resulted area under ROC curve value of 0.97 (with 95% confidence interval 0.92-1.0 and p-value <0.0001) suggested that the circulatory GHR levels have significant potential to serve as a better preclinical indicator of disease activity in RA and may be a reliable clinical biomarker for monitoring the treatment response in such preclinical models.     Background: In pediatric age the prevalence of obesity is high. Obese children who do not have other risk factors than excess weight have been defined as "metabolically healthy obese" (MHO). Aim: The aim of this study is to evaluate, in a population of obese children, the prevalence of the MHO and "metabolically unhealthy obese" (MUO) phenotype. Furthermore, we evaluated the distribution of Uric Acid, HOMA index and Waist-Height ratio (W-Hr) in the MHO and MUO sub-groups and the impact of these non-traditional risk factors on the probability to be MUO. Methods:In 1201 obese children and adolescents [54% males, age (±SD) 11.9 (±3.0) years] weight, height, waist circumference, systolic (SBP) and diastolic (DBP) blood pressure, pubertal status, glucose, insulin, HDL cholesterol, triglycerides and Uric Acid serum values were assessed. MUO phenotype was defined as the presence of at least one of the following risk factors: SBP or DBP ≥ 90th percentile, glycaemia ≥ 100 mg/dl, HDL cholesterol <40 mg/dl, triglycerides ≥100 mg/dl (children <10 years) or ≥130 mg/dl (children ≥10 years). A multivariate logistic regression analysis was used to estimate the association between MUO phenotype and non-traditional cardiovascular risk factors. Results: The prevalence of the MUO status was high (61%). MUO subjects were more often male, older and pubertal (p < 0.001). The levels of the three non-traditional risk factors were significantly higher in MUO children compared to MHO children (p < 0.001) and all of them were independent predictors of the fact of being MUO [OR 1.41 (95% CI 1.24-1.69); 1.15 (95% CI 1.06-1.23) and 1.03 (95% CI1.01-1.05) for Uric Acid, HOMA index and W-Hr, respectively]. About 15% of MHO subjects had serum Uric Acid, HOMA index and W-Hr values within the highest quartile of the study population. Conclusion: The prevalence of MUO subjects in a large pediatric population is high and serum Uric Acid, HOMA index and W-Hr values are independent predictors of the probability of being MUO. A non-negligible percentage of subjects MHO has high values of all three non-traditional risk factors.
