    Hipofizė yra pagrindinė endokrininės sistemos reguliavimo liauka, kuri per hormonų sekreciją perduoda pogumburio signalus tiksliniams organams. Posmegeninė liauka, išskirdama hormonus, reguliuoja gyvybiškai svarbias fiziologines funkcijas: augimą, dauginimąsi, medžiagų apykaitą ir reakciją į stresą. Hipofizės pažeidimai gali sukelti gyvybei pavojingą homeostazės disbalansą. Sutrikus homeostazei, gali vystytis hipofizės adenomos (HA). Jos dažniausiai yra gerybiniai navikai, atsirandantys iš priekinės hipofizės liaukos epitelio ląstelių. HA yra vienas labiausiai paplitusių centrinės nervų sistemos navikų. Pagal hormonų sekreciją HA yra klasifikuojama į nesekretuojančią (nefunkcionuojančią) ir sekretuojančią (funkciškai aktyvią). Hormonų perteklius, išskiriamas HA, gali sukelti klinikinius sindromus, tokius kaip hiperprolaktinemija (dėl perteklinio prolaktino sekrecijos), akromegalija (dėl augimo hormono hipersekrecijos) ir Kušingo liga (dėl perteklinio adrenokortikotropinio hormono gamybos). Remiantis tyrimų duomenimis, nustatyta, kad HA populiacinis paplitimas nustatomas 80 iš 100 000 asmenų. HA pasireiškimui turi įtakos ne tik somatiniai ar lytinių ląstelių defektai, bet ir aplinkos veiksniai. Tiksli HA atsiradimo priežastis vis dar nėra aiški, tačiau žinoma, kad nedidelė dalis (~5 %) HA atsiranda dėl paveldimų sindromų, kuriuos sukelia geno pokyčiai. Atsižvelgiant į aplinkos veiksnių įtaką ir genetinius pakitimus, kurie turi įtakos HA pasireiškimui, galima ieškoti naujų genetinių žymenų šiai ligai nustatyti. Vienas iš rizikos veiksnių, skatinantis HA išsivystymą ir jos agresyvumą, yra alkoholio vartojimas. Nesaikingas alkoholio vartojimas daro įtaką ne tik HA išsivystymui, bet ir kitų vėžinių ligų pasireiškimui. Neseniai atliktame tyrime buvo atkreiptas dėmesys į kartaus skonio 2 receptorių šeimos 16 nario (TAS2R16) vieno nukleotido polimorfizmų (VNP) sąsają su storosios žarnos vėžiu, kadangi nustatyta, kad šio geno VNP didina priklausomybę nuo alkoholio. Remdamiesi TAS2R16 VNP sąsajomis su vėžiniais susirgimais, pasirinkome įvertinti TAS2R16 geno (rs978739, rs1357949) polimorfizmų sąsajas su HA. The pituitary gland is the primary regulatory gland in the endocrine system transmitting signals from the hypothalamus to target organs through hormone release. Through the release of hormones, the pituitary gland regulates vital physiological functions: growth, reproduction, metabolism, and the stress response. Disorders in the pituitary gland can cause damage, such as pituitary adenomas (PA). PA is the most common benign tumor arising from epithelial cells of the anterior pituitary gland. PA is one of the most common tumors of the central nervous system. PA is differentiated by hormone secretion into non-secretory (non- functional) and secretory (functionally active). Excess hormones secreted by PA can cause clinical syndromes, such as hyperprolactinemia (caused by excessive secretion of prolactin), acromegaly (due to excess growth hormone), and Cushing’s syndrome (due to excessive production of the adrenocorticotropic hormone ACTH)). Studies show that the prevalence of adenomas in the population is approximately 80/100,000. The occurrence of PA is influenced not only by somatic or germ cell defects but also by environmental factors. The exact cause of PA is still unclear, but it is known that a small part (~5%) of PA due to hereditary syndromes is caused by gene changes. Considering the influence of environmental factors and genetic alterations that affect the occurrence of PA, new genetic markers can be searched to detect this disease. One of the risk factors contributing to the development of PA and its aggressiveness is alcohol consumption. Excessive alcohol consumption affects not only the development of PA but also the manifestation of other cancers. A recent study has investigated the association of TAS2R16 gene single nucleotide polymorphisms (SNPs) with colon cancer, as SNPs in this gene have been found to increase alcohol dependence. Based on the association of TAS2R16 SNPs with cancer, we decided to investigate the association of TAS2R16 gene (rs978739, rs1357949) polymorphisms with PA.

    Cassia glauca Lam. is a medicinal plant, belonging to the family Caesalpiniaceae. This plant is locally known as pyi-pan-nyo. The plant samples such as leaves are collected from Pan-Taw-Yoe village, Kyaunggon Township, Ayeyarwady Region during the flowering and fruiting period. The morphological characters and phytochemical test according to the methods of Marini Bettolo (1981), Trease and Evans (2002) and British Pharmacopoeia (1968). The plant is used as medicine in the treatment of diabetes, gonorrhoea, dysuria, dysentery, fever, colic and cough.

    Multifocal motor neuropathy is a rare motor neuron disease characterised by a progressive, gradual loss of muscle strength, although sensory functions are preserved. The worldwide frequency of this disease is estimated at 1 person per 100,000 people. MMN is diagnosed when there are characteristic clinical signs, electroneuromyography test results support the clinical findings, and other possible diagnoses are excluded. To prove the progressive course of the disease, the patient must be under long-term medical supervision, since the deteriorating state of the patient may imitate other diseases affecting the motor neuron, such as amyotrophic lateral sclerosis, chronic inflammatory demyelinating polyneuropathy, spinal muscular atrophy, and other. We present the clinical case of a 71-year-old patient who presented to the Neurology Department of the Hospital of Lithuanian University of Health Sciences Kauno Klinikos. She complained of pain in her right hip and progressing weakness of the right foot which had lasted for a year. After laboratory tests, long-term supervision, and exclusion of other diagnoses, the patient was diagnosed with an atypical type of multifocal motor neuropathy responsive to treatment with intravenous immunoglobulin. Daugiažidininė motorinė neuropatija diagnozuojama esant charakteringiems klinikiniams požymiams, kurie paremti elektroneuromiografijos tyrimo rezultatais ir kitų panašių ligų atmetimu. Tam tikrais atvejais, ligai progresuojant, reikalingas paciento stebėjimas, kadangi simptomai dažnai būdingi ir šoninei amiotrofinei sklerozei, lėtinei uždegiminei demielinizuojančiai polineuropatijai, spinalinei raumenų atrofijai ar kitoms ligoms. Mes pristatome klinikinį atvejį: apie 71 metų pacientė atvyko į Lietuvos sveikatos mokslų universiteto ligoninės Kauno klinikos Neurologijos ambulatorijos skyrių dėl metus trunkančio skausmo strėnose, klubuose ir atsiradusio dešinės kojos silpnumo. Po atliktų tyrimų, ilgai trukusio stebėjimo, atmetus kitas ligas, pacientei nustatyta netipinės eigos veiksmingai intraveniniu imunoglobulinu gydyta daugiažidininės motorinės neuropatijos forma.

    Hipofizė yra pagrindinė endokrininės sistemos reguliavimo liauka, kuri per hormonų sekreciją perduoda pogumburio signalus tiksliniams organams. Posmegeninė liauka, išskirdama hormonus, reguliuoja gyvybiškai svarbias fiziologines funkcijas: augimą, dauginimąsi, medžiagų apykaitą ir reakciją į stresą. Hipofizės pažeidimai gali sukelti gyvybei pavojingą homeostazės disbalansą. Sutrikus homeostazei, gali vystytis hipofizės adenomos (HA). Jos dažniausiai yra gerybiniai navikai, atsirandantys iš priekinės hipofizės liaukos epitelio ląstelių. HA yra vienas labiausiai paplitusių centrinės nervų sistemos navikų. Pagal hormonų sekreciją HA yra klasifikuojama į nesekretuojančią (nefunkcionuojančią) ir sekretuojančią (funkciškai aktyvią). Hormonų perteklius, išskiriamas HA, gali sukelti klinikinius sindromus, tokius kaip hiperprolaktinemija (dėl perteklinio prolaktino sekrecijos), akromegalija (dėl augimo hormono hipersekrecijos) ir Kušingo liga (dėl perteklinio adrenokortikotropinio hormono gamybos). Remiantis tyrimų duomenimis, nustatyta, kad HA populiacinis paplitimas nustatomas 80 iš 100 000 asmenų. HA pasireiškimui turi įtakos ne tik somatiniai ar lytinių ląstelių defektai, bet ir aplinkos veiksniai. Tiksli HA atsiradimo priežastis vis dar nėra aiški, tačiau žinoma, kad nedidelė dalis (~5 %) HA atsiranda dėl paveldimų sindromų, kuriuos sukelia geno pokyčiai. Atsižvelgiant į aplinkos veiksnių įtaką ir genetinius pakitimus, kurie turi įtakos HA pasireiškimui, galima ieškoti naujų genetinių žymenų šiai ligai nustatyti. Vienas iš rizikos veiksnių, skatinantis HA išsivystymą ir jos agresyvumą, yra alkoholio vartojimas. Nesaikingas alkoholio vartojimas daro įtaką ne tik HA išsivystymui, bet ir kitų vėžinių ligų pasireiškimui. Neseniai atliktame tyrime buvo atkreiptas dėmesys į kartaus skonio 2 receptorių šeimos 16 nario (TAS2R16) vieno nukleotido polimorfizmų (VNP) sąsają su storosios žarnos vėžiu, kadangi nustatyta, kad šio geno VNP didina priklausomybę nuo alkoholio. Remdamiesi TAS2R16 VNP sąsajomis su vėžiniais susirgimais, pasirinkome įvertinti TAS2R16 geno (rs978739, rs1357949) polimorfizmų sąsajas su HA. The pituitary gland is the primary regulatory gland in the endocrine system transmitting signals from the hypothalamus to target organs through hormone release. Through the release of hormones, the pituitary gland regulates vital physiological functions: growth, reproduction, metabolism, and the stress response. Disorders in the pituitary gland can cause damage, such as pituitary adenomas (PA). PA is the most common benign tumor arising from epithelial cells of the anterior pituitary gland. PA is one of the most common tumors of the central nervous system. PA is differentiated by hormone secretion into non-secretory (non- functional) and secretory (functionally active). Excess hormones secreted by PA can cause clinical syndromes, such as hyperprolactinemia (caused by excessive secretion of prolactin), acromegaly (due to excess growth hormone), and Cushing’s syndrome (due to excessive production of the adrenocorticotropic hormone ACTH)). Studies show that the prevalence of adenomas in the population is approximately 80/100,000. The occurrence of PA is influenced not only by somatic or germ cell defects but also by environmental factors. The exact cause of PA is still unclear, but it is known that a small part (~5%) of PA due to hereditary syndromes is caused by gene changes. Considering the influence of environmental factors and genetic alterations that affect the occurrence of PA, new genetic markers can be searched to detect this disease. One of the risk factors contributing to the development of PA and its aggressiveness is alcohol consumption. Excessive alcohol consumption affects not only the development of PA but also the manifestation of other cancers. A recent study has investigated the association of TAS2R16 gene single nucleotide polymorphisms (SNPs) with colon cancer, as SNPs in this gene have been found to increase alcohol dependence. Based on the association of TAS2R16 SNPs with cancer, we decided to investigate the association of TAS2R16 gene (rs978739, rs1357949) polymorphisms with PA.

    Cassia glauca Lam. is a medicinal plant, belonging to the family Caesalpiniaceae. This plant is locally known as pyi-pan-nyo. The plant samples such as leaves are collected from Pan-Taw-Yoe village, Kyaunggon Township, Ayeyarwady Region during the flowering and fruiting period. The morphological characters and phytochemical test according to the methods of Marini Bettolo (1981), Trease and Evans (2002) and British Pharmacopoeia (1968). The plant is used as medicine in the treatment of diabetes, gonorrhoea, dysuria, dysentery, fever, colic and cough.

    Multifocal motor neuropathy is a rare motor neuron disease characterised by a progressive, gradual loss of muscle strength, although sensory functions are preserved. The worldwide frequency of this disease is estimated at 1 person per 100,000 people. MMN is diagnosed when there are characteristic clinical signs, electroneuromyography test results support the clinical findings, and other possible diagnoses are excluded. To prove the progressive course of the disease, the patient must be under long-term medical supervision, since the deteriorating state of the patient may imitate other diseases affecting the motor neuron, such as amyotrophic lateral sclerosis, chronic inflammatory demyelinating polyneuropathy, spinal muscular atrophy, and other. We present the clinical case of a 71-year-old patient who presented to the Neurology Department of the Hospital of Lithuanian University of Health Sciences Kauno Klinikos. She complained of pain in her right hip and progressing weakness of the right foot which had lasted for a year. After laboratory tests, long-term supervision, and exclusion of other diagnoses, the patient was diagnosed with an atypical type of multifocal motor neuropathy responsive to treatment with intravenous immunoglobulin. Daugiažidininė motorinė neuropatija diagnozuojama esant charakteringiems klinikiniams požymiams, kurie paremti elektroneuromiografijos tyrimo rezultatais ir kitų panašių ligų atmetimu. Tam tikrais atvejais, ligai progresuojant, reikalingas paciento stebėjimas, kadangi simptomai dažnai būdingi ir šoninei amiotrofinei sklerozei, lėtinei uždegiminei demielinizuojančiai polineuropatijai, spinalinei raumenų atrofijai ar kitoms ligoms. Mes pristatome klinikinį atvejį: apie 71 metų pacientė atvyko į Lietuvos sveikatos mokslų universiteto ligoninės Kauno klinikos Neurologijos ambulatorijos skyrių dėl metus trunkančio skausmo strėnose, klubuose ir atsiradusio dešinės kojos silpnumo. Po atliktų tyrimų, ilgai trukusio stebėjimo, atmetus kitas ligas, pacientei nustatyta netipinės eigos veiksmingai intraveniniu imunoglobulinu gydyta daugiažidininės motorinės neuropatijos forma.
