    Resumen: Este escrito describe y analiza la experiencia de inserción de las Franciscanas Misioneras de la Madre del Divino Pastor. Estas religiosas hacen parte del grupo de las que se lanzaron a esta experiencia a principios de los 80s capitalizando la de aquellas pioneras que lo hicieron en los 60s-70s. El escrito analiza la presencia de esta congregación en el país desde su arribo, el camino que las llevó a asumir la inserción, el tipo de presencia entre los pobres, los desafíos que enfrentaron y el balance de la experiencia. Se argumenta que la continuidad en el presente de comunidades de inserción obedece al modelo que lograron instrumentar que les permitió seguir articulando con sus colegios, con una variedad de redes públicas y privadas, y mantener un espacio propio, interrelacionado con la iglesia local, pero con suficiente autonomía de acción. Las fuentes de datos son archivos de la congregación, entrevistas efectuadas en 2017-2018 a 19 religiosas de la institución, a laicos vinculados con sus obras, y al trabajo de campo etnográfico en seis de sus comunidades.

    The obesity is considered the 21st century pandemic and its prevalence in the world is increasing, this pattern is observed in Colombia according to the results of the ENSIN 2010 and 2015. This disease is caused by an energy imbalance lead by a low energy expenditure in sedentary individuals and overconsumption of high energy foods, which can be considered as an addictive like behavior in specific conditions determined in the Yale food addiction scale. The neurobiological and behavioral changes exhibited in obese people with food addiction can be associated to the activity of the endocannabinoid system in the central nervous system, given the fact that obese population have high rates of consumption of food rich in linoleic acid or 6 omega, which derivative form, the arachidonic acid is the main precursor of endocannabinoids, compounds that have increased plasmatic levels in obese people. The main purpose of this study is to determine the association between plasmatic levels of arachidonic acid and diagnostic criteria of food addiction in obese adults after inducing changes in the essential fatty acids intake. This thesis consists in a prospective quasi-experimental with clinical trial model, non-randomized, single-blind, crossover pilot study, conducted in obese adults whom were offered two nutritional treatments of obesity, one of these with a decrease in the omega 6 consumption and an increase in the omega 3 intake. The main findings indicate a positive correlation between arachidonic acid plasmatic levels and the scoring in the food addiction scale in individuals exposed to the experimental nutritional treatment. These results are held by the statistically significant decrease in plasmatic levels of arachidonic acid, the score in the food addiction scale, the body weight and the triglycerides plasma concentration found in the participants who had received the intervention that modified the consumption of essential fatty acids. Other variables such as body fat percentage and plasmatic levels of total cholesterol, HDL and LDL did not exhibit expected behaviors according to the literature, so it is thought that the time of exposure to interventions was not enough in order to get significant changes in such La obesidad es considerada la pandemia del siglo XXI y sus prevalencias en el mundo van en aumento, lo cual coincide con el patrón reportado en Colombia por la ENSIN 2010 y 2015. Esta enfermedad tiene lugar cuando se genera un desbalance energético producto de un bajo gasto de energía en individuos inactivos físicamente con un consumo excesivo de alimentos ricos en energía, el cual puede considerarse como una conducta adictiva en condiciones específicas determinadas en la escala de adicción a la comida de Yale. Los diferentes cambios neurobiológicos y conductuales evidenciados en individuos obesos con adicción a la comida pueden estar asociados a la actividad del sistema endocannabinoide en el sistema nervioso central, debido a que los alimentos con un mayor consumo por parte de la población obesa tienen un alto aporte de Omega 6, a partir del que se produce el ácido araquidónico, que a su vez es el principal precursor de los cannabinoides endógenos, compuestos que suelen encontrarse aumentados a nivel plasmático en personas con obesidad. De manera que el objetivo general del estudio consiste en establecer una relación entre niveles plasmáticos de ácido araquidónico y la presencia de criterios de la adicción a la comida en adultos obesos tras efectuar cambios a nivel de la ingesta dietaría de alimentos fuente de Omegas 6 y alimentos fuente de 3. La presente tesis consiste en un estudio piloto cuasiexperimental prospectivo con modelo de ensayo clínico, no aleatorizado, simple ciego, cruzado en dos tiempos, efectuado en adultos obesos a quienes les son ofrecidos dos tratamientos nutricionales para el manejo de la obesidad, uno de estos con una disminución en el consumo de Omega 6 y un aumento en el aporte de Omega 3, encontrando una correlación positiva entre los niveles plasmáticos de ácido araquidónico y el puntaje en la escala de adicción a la comida en individuos tras su exposición al tratamiento experimental. Tales hallazgos se soportan en la disminución estadísticamente significativa de los niveles plasmáticos de ácido araquidónico, el puntaje en la escala de adicción a la comida, el peso corporal y las concentraciones plasmáticas de triglicéridos en los participantes que habían recibido la intervención dietaría con modificación en el consumo de ácidos grasos esenciales. Otras variables como el porcentaje de grasa corporal y los niveles plasmáticos de colesterol total, HDL y LDL no presentaron comportamientos esperados según la literatura, por lo que se piensa que el tiempo de exposición a las intervenciones no fue suficiente en orden de obtener cambios significativos en estas.     El objetivo de este artículo es establecer las bases del mandato de Obama como primer e-president de la historia, concepto que implica en sí mismo un alto grado de novedad. Para ello, primero se describe cómo fue la comunicación política de las administraciones norteamericanas del siglo XX y principios del XXI hasta 2008. Se aporta un marco teórico relacionado directamente con la comunicación política y con los conceptos de e-democracia, tecnologías de la información y la comunicación, ciber democracia, e-administración, etc. Se establecen y analizan entonces los elementos disruptivos del mandato de Obama como agente del cambio desde antes de su periodo presidencial, como candidato, durante su mandato, y después como expresidente desde su fundación. Este es un artículo basado en la consulta de libros, revistas, artículos académicos, así como de las webs presidenciales disponibles en la actualidad. Los resultados establecen un principio de liderazgo que supera ampliamente las redes sociales. The aim of this article is to establish the basis of Obama’s mandate as the !rst e-president in history, a concept that in itself implies a high degree of novelty. To do so, we !rst describe the political communication of the American administrations of the 20th century and the beginning of the 21st century until 2008. A theoretical framework directly related to political communication and the concepts of e-democracy, information and communication technologies, cyberdemocracy, e-administration, etc. is provided. It then establishes and analyzes the disruptive elements of Obama’s mandate as an agent of change from before his presidential term, as a candidate, during his term, and then as an ex-president since his Foundation. This is an article based on consultation of books, journals, academic articles and currently available presidential websites. The results establish a leadership principle that far surpasses social networks.

    El Rituximab (Rx) es un anticuerpo monoclonal quimérico que se une al CD20 expresado en la superficie de los linfocitos B (LB). Aunque su principal aplicación es en el tratamiento de leucemias y linfomas B, en los últimos años ha dado muy buenos resultados en el tratamiento de enfermedades autoinmunes, especialmente en aquellas donde el daño está mediado por autoanticuerpos. La hipótesis aceptada es que la intensa depleción de los LB producida por el Rx conlleva la eliminación de los LB autorreactivos, que se asocia a una reducción del título de autoanticuerpos y por tanto una mejoría clínica. Sin embargo, aunque su eficacia en general es significativa, existe una gran variabilidad en la respuesta de los pacientes, sin que podamos predecir cuál va a ser, ni cuándo se va a producir una recaída. Los principales trabajos que se ocupan de la monitorización post-Rx utilizan la citometría de flujo como herramienta para el análisis de las subpoblaciones B post-Rx y su correlación con la clínica del paciente. Pero la baja sensibilidad aplicada y la baja especificidad a la hora de definir el inmunofenotipo de las diferentes subpoblaciones hacen que exista una gran variabilidad en las conclusiones en cuanto al cómo, cuándo y qué tipo de células analizar. Es por ello que la repoblación B post-Rx no se monitoriza, decidiéndose empíricamente la pauta de administración del fármaco en función de criterios clínicos. Teniendo en cuenta el coste del Rx así como sus potenciales efectos secundarios, sería deseable contar con un sistema que objetivara el efecto del fármaco, para así ajustar mejor la dosis y la pauta terapéutica en cada paciente. Por ello creemos que, la aplicación de técnicas de citometría de flujo ultrasensible, junto con el estudio exhaustivo de subpoblaciones B en diferentes tejidos, permitirá detectar poblaciones mínimas en SP que permanecen indetectables con las técnicas utilizadas hasta el momento. Esto supondrá un avance significativo a la hora de monitorizar las características de la repoblación B post-Rituximab en enfermedades autoinmunes. Para ello desarrollamos un protocolo de citometría de alta sensibilidad, y definimos la combinación óptima de marcadores para el análisis de subpoblaciones B, analizando la diferenciación B completa desde la médula ósea hasta los órganos linfoides secundarios como ganglio, amígdala y bazo, además de la sangre periférica (SP). También se incluyó el análisis de calidad de las muestras de MO frecuentemente hemodiluidas en los estudios de citometría de flujo. Aplicando el protocolo de citometría de alta sensibilidad y el panel de marcadores definidos, estudiamos 72 muestras de SP post-Rx de 33 pacientes con enfermedades autoinmunes donde identificamos un patrón común de reconstitución B en el que definimos 4 etapas sucesivas; fase de depleción, inicio de reconstitución, repoblación precoz y repoblación tardía. Demostramos que una clasificación por etapas basada en la distribución de subpoblaciones ilustra mejor la realidad funcional del compartimento B, mejor que el tiempo post-Rx y mejor que la cantidad total de LB. Con ello pudimos definir las características para una correcta monitorización por citometría post-Rx en pacientes con enfermedades autoinmunes: Primero, el panel de marcadores debe permitir la identificación fiable de LB memoria CD27+ que han realizado cambio de isotipo, junto a la de plasmablastos, ambas relacionadas con la clínica de la enfermedad, por lo que debe incluir los marcadores CD19, CD38, CD24, CD27, IgM e IgD, además del marcador de exclusión CD3. Segundo, el análisis debe realizarse en la etapa de depleción, en torno a los 6 meses tras el tratamiento, antes de que se inicie la reconstitución B ya que puede enmascarar los resultados. Finalmente, se requiere de la adquisición de un mínimo de 3 millones de leucocitos para alcanzar un límite de sensibilidad suficiente de 10-5, ya que los LB son una población residual tras el tratamiento con Rx.

    Este trabajo tiene por objeto demostrar que existe una asociación entre la enfermedad venosa crónica (EVC) y la onicodistrofia y evaluar la frecuencia con la que los pacientes que padecen EVC presentan onicodistrofia en los pies. Para ello, se realizó en primer lugar una prueba piloto a partir de una muestra constituida por 40 pacientes. Posteriormente, se amplió la muestra hasta los 83 pacientes que se analizaron en el trabajo definitivo. Los participantes se captaron entre los pacientes que acudieron al Servicio de Angiología, Cirugía Vascular y Endovascular del Hospital de la Santa Creu i Sant Pau de Barcelona, previa lectura y firma del documento de consentimiento informado (CI) y la Hoja de información del paciente y tras haber declarado que participaban voluntariamente en la investigación. El estudio estuvo sujeto al seguimiento y recibió la aprobación del Comité Ético de Investigación Clínica del centro médico. Los participantes debían tener entre 40 y 85 años y padecer enfermedad venosa crónica en una o ambas piernas. Se desestimó a los pacientes que llevaban prótesis o cosméticos ungueales, presentaban alteraciones hereditarias en el cuerpo ungueal, amputaciones (en la pierna o en el pie) o intervenciones realizadas en alguna uña del pie durante el último año, así como aquellos cuya onicodistrofia era atribuible y guardaba una relación clara y directa con medicamentos, tumores, infecciones ungueales, deformaciones digitales, problemas biomecánicos, enfermedad articular degenerativa evolucionada, uso habitual de calzado inapropiado o mal ajustado, alteraciones permanentes en las uñas producidas por un procedimiento quirúrgico, enfermedades dermatológicas, traumatismos en las uñas y la presencia de un segundo dedo del pie más largo que el primero. Se desestimó asimismo la participación de las personas con síndrome postrombótico y de las que habían sido sometidas a cualquier intervención quirúrgica invasiva en una extremidad inferior. También quedaron excluidos del estudio los pacientes que, a pesar de cumplir los requisitos, rehusaron participar o se negaron a firmar el documento de CI, al igual que las personas que, por cualquier dolencia médica o trastorno psiquiátrico, a juicio de la investigadora, no comprendían el motivo de su selección o no estaban capacitadas para otorgar el CI para participar en el estudio. Los pacientes aptos para la investigación fueron visitados y sometidos a un eco-Doppler venoso por el facultativo del centro, tras lo cual la investigadora recabó los datos que resultaban de interés para el estudio y obtuvo una fotografía de los dos pies de los participantes. La participación en este estudio no supuso ningún riesgo, molestia ni inconveniente de ningún tipo para los pacientes. Este análisis pretende contribuir a la prevención de la onicodistrofia y dotar de una mayor precisión su diagnóstico con vistas a que se pueda establecer un tratamiento más específico y riguroso en el momento en que el facultativo vea los pies, observe el historial clínico o verifique los antecedentes de EVC del paciente. The purpose of this research is to show that there is an association between chronic venous disease (CVD) and onychodystrophy and to assess the frequency with which patients with CVD suffer onychodystrophy in their feet. To this end, a pilot study with a sample of 40 patients was performed in the first place. The sample was later expanded to the 83 patients analyzed in the definitive research. Participants were recruited amongst the patients who went to the Service of Angiology, Vascular and Endovascular Surgery of the Hospital de la Santa Creu i Sant Pau in Barcelona, Spain, after they had read and signed the informed consent document and the patient information sheet and they had stated that their participation was voluntary. The research was monitored and approved by the Clinical Research Ethics Committee of the above mentioned medical center. Participants had to be between 40 and 85 years of age and suffer CVD in one or both legs. Patients with nail prosthesis or cosmetics, hereditary alterations in the nail body, amputations (in legs or feet) or interventions performed in a toe nail during the past year were dismissed, as well as those suffering from onychodystrophy attributable to and clearly and directly related to drugs, tumors, nail infections, finger deformations, biomechanical problems, evolved degenerative joint disease, frequent use of inappropriate or badly adjusted footwear, permanent nail alterations caused by a surgical procedure, skin disease, nail injuries and the presence of a second toe which is longer than the first. Patients suffering from postthrombotic syndrome and those who had undergone any invasive surgical operation in a lower limb were also denied participation. Patients who, despite fulfilling the requirements, refused to take part in the study or to sign the informed consent document were also excluded, as well as people who, as a result of any medical condition or psychiatric disorder, did not understand, in the researcher's view, the reason for their selection or were unable to give their informed consent to taking part in the study. The patients who were eligible for the study were explored by the physician of the above mentioned medical center and underwent a venous echo-Doppler. Later, the researcher collected data of interest for the study and took a photograph of both feet of the participants. Participation in this study did not involve any risk, annoyance or inconvenience of any sort for the patients. This analysis is aimed at contributing to the prevention of onychodystrophy and to a greater precision in its diagnosis with a view to establishing a more specific and rigorous treatment when the physician sees the patient's feet, examines their clinical records or checks their CVD background.

    Resumen: Este escrito describe y analiza la experiencia de inserción de las Franciscanas Misioneras de la Madre del Divino Pastor. Estas religiosas hacen parte del grupo de las que se lanzaron a esta experiencia a principios de los 80s capitalizando la de aquellas pioneras que lo hicieron en los 60s-70s. El escrito analiza la presencia de esta congregación en el país desde su arribo, el camino que las llevó a asumir la inserción, el tipo de presencia entre los pobres, los desafíos que enfrentaron y el balance de la experiencia. Se argumenta que la continuidad en el presente de comunidades de inserción obedece al modelo que lograron instrumentar que les permitió seguir articulando con sus colegios, con una variedad de redes públicas y privadas, y mantener un espacio propio, interrelacionado con la iglesia local, pero con suficiente autonomía de acción. Las fuentes de datos son archivos de la congregación, entrevistas efectuadas en 2017-2018 a 19 religiosas de la institución, a laicos vinculados con sus obras, y al trabajo de campo etnográfico en seis de sus comunidades.

    The obesity is considered the 21st century pandemic and its prevalence in the world is increasing, this pattern is observed in Colombia according to the results of the ENSIN 2010 and 2015. This disease is caused by an energy imbalance lead by a low energy expenditure in sedentary individuals and overconsumption of high energy foods, which can be considered as an addictive like behavior in specific conditions determined in the Yale food addiction scale. The neurobiological and behavioral changes exhibited in obese people with food addiction can be associated to the activity of the endocannabinoid system in the central nervous system, given the fact that obese population have high rates of consumption of food rich in linoleic acid or 6 omega, which derivative form, the arachidonic acid is the main precursor of endocannabinoids, compounds that have increased plasmatic levels in obese people. The main purpose of this study is to determine the association between plasmatic levels of arachidonic acid and diagnostic criteria of food addiction in obese adults after inducing changes in the essential fatty acids intake. This thesis consists in a prospective quasi-experimental with clinical trial model, non-randomized, single-blind, crossover pilot study, conducted in obese adults whom were offered two nutritional treatments of obesity, one of these with a decrease in the omega 6 consumption and an increase in the omega 3 intake. The main findings indicate a positive correlation between arachidonic acid plasmatic levels and the scoring in the food addiction scale in individuals exposed to the experimental nutritional treatment. These results are held by the statistically significant decrease in plasmatic levels of arachidonic acid, the score in the food addiction scale, the body weight and the triglycerides plasma concentration found in the participants who had received the intervention that modified the consumption of essential fatty acids. Other variables such as body fat percentage and plasmatic levels of total cholesterol, HDL and LDL did not exhibit expected behaviors according to the literature, so it is thought that the time of exposure to interventions was not enough in order to get significant changes in such La obesidad es considerada la pandemia del siglo XXI y sus prevalencias en el mundo van en aumento, lo cual coincide con el patrón reportado en Colombia por la ENSIN 2010 y 2015. Esta enfermedad tiene lugar cuando se genera un desbalance energético producto de un bajo gasto de energía en individuos inactivos físicamente con un consumo excesivo de alimentos ricos en energía, el cual puede considerarse como una conducta adictiva en condiciones específicas determinadas en la escala de adicción a la comida de Yale. Los diferentes cambios neurobiológicos y conductuales evidenciados en individuos obesos con adicción a la comida pueden estar asociados a la actividad del sistema endocannabinoide en el sistema nervioso central, debido a que los alimentos con un mayor consumo por parte de la población obesa tienen un alto aporte de Omega 6, a partir del que se produce el ácido araquidónico, que a su vez es el principal precursor de los cannabinoides endógenos, compuestos que suelen encontrarse aumentados a nivel plasmático en personas con obesidad. De manera que el objetivo general del estudio consiste en establecer una relación entre niveles plasmáticos de ácido araquidónico y la presencia de criterios de la adicción a la comida en adultos obesos tras efectuar cambios a nivel de la ingesta dietaría de alimentos fuente de Omegas 6 y alimentos fuente de 3. La presente tesis consiste en un estudio piloto cuasiexperimental prospectivo con modelo de ensayo clínico, no aleatorizado, simple ciego, cruzado en dos tiempos, efectuado en adultos obesos a quienes les son ofrecidos dos tratamientos nutricionales para el manejo de la obesidad, uno de estos con una disminución en el consumo de Omega 6 y un aumento en el aporte de Omega 3, encontrando una correlación positiva entre los niveles plasmáticos de ácido araquidónico y el puntaje en la escala de adicción a la comida en individuos tras su exposición al tratamiento experimental. Tales hallazgos se soportan en la disminución estadísticamente significativa de los niveles plasmáticos de ácido araquidónico, el puntaje en la escala de adicción a la comida, el peso corporal y las concentraciones plasmáticas de triglicéridos en los participantes que habían recibido la intervención dietaría con modificación en el consumo de ácidos grasos esenciales. Otras variables como el porcentaje de grasa corporal y los niveles plasmáticos de colesterol total, HDL y LDL no presentaron comportamientos esperados según la literatura, por lo que se piensa que el tiempo de exposición a las intervenciones no fue suficiente en orden de obtener cambios significativos en estas.
